Additional requirements for licensed<strong>asbestos</strong> removal• the name, <strong>in</strong>clud<strong>in</strong>g registered bus<strong>in</strong>ess or corporate name, and address ofthe workplace and the type of workplace where the <strong>asbestos</strong> removal workwill be performed <strong>in</strong>clud<strong>in</strong>g the specific location if it is a large workplace• the date of notification• the commencement date and estimated duration of the <strong>asbestos</strong>removal work• whether the <strong>asbestos</strong> is friable ACM or non-friable ACM• if friable ACM is to be removed, details of the way that the area wherethe <strong>asbestos</strong> removal work is to be performed will be enclosed• the type of ACM• the estimated quantity of <strong>asbestos</strong> to be removed• the number of employees who will perform the <strong>asbestos</strong> removal work• details of tra<strong>in</strong><strong>in</strong>g and experience of those <strong>in</strong>dividual employees, if differentto the <strong>in</strong>formation notified previously• the date of any <strong>asbestos</strong> register or employer’s <strong>asbestos</strong> register usedto prepare the <strong>asbestos</strong> control plan.A pro forma notification can be downloaded at worksafe.vic.gov.au.245. Before commenc<strong>in</strong>g <strong>asbestos</strong> removal work, the licensed removalist mustalso give a copy of the notification to the person who commissioned theremoval work.What if a change occurs to <strong>in</strong>formation provided <strong>in</strong> the notification?246. If a change occurs to any <strong>in</strong>formation <strong>in</strong> the notification provided to <strong>WorkSafe</strong>,the licensed removalist must advise <strong>WorkSafe</strong> <strong>in</strong> writ<strong>in</strong>g of that change as soonas possible after they become aware a change has occurred. However, thisrequirement does not apply to any change of supervisor (if a change doesoccur, an additional five days notification is not required).247. If the licensed removalist has changed, the new removalist needs to providea new notification with a m<strong>in</strong>imum five days notice.What happens when an unexpected situation arises?248. An unexpected situation is:• a sudden unexpected event, <strong>in</strong>clud<strong>in</strong>g non-rout<strong>in</strong>e failures of equipment, thatmay result <strong>in</strong> a person be<strong>in</strong>g exposed to airborne <strong>asbestos</strong> fibres (eg a burstpipe that was lagged with <strong>asbestos</strong> or a forklift crash<strong>in</strong>g <strong>in</strong>to an <strong>asbestos</strong>cement sheet wall), or• an unexpected breakdown of an essential service (<strong>in</strong>clud<strong>in</strong>g gas, water,sewerage, electricity and telecommunications) that requires immediaterectification to enable cont<strong>in</strong>uance of that service (eg a breakdown of hotwater system that requires the removal of <strong>asbestos</strong>-conta<strong>in</strong><strong>in</strong>g <strong>in</strong>sulationor the replacement of burst <strong>asbestos</strong> cement water pipes to enablecont<strong>in</strong>uance of the service).249. In these situations the licensed removalist must notify <strong>WorkSafe</strong> <strong>in</strong> writ<strong>in</strong>g notlater than 24 hours after commenc<strong>in</strong>g the removal work.
