Standard’s Text Pre-described Finding PDFcodethe holistic descriptors at Section 2 and with the ICAOOperational Level (Level 4) of the ICAO LanguageProficiency Rating Scale in Attachment A.A1-5.1.1.2 The following details shall appear on the licence:I) Name of State (in bold type);II) Title of licence (in very bold type);III) Serial number of the licence, in Arabic numerals,given by the authority issuing the licence;IV) Name of holder in full (in Roman alphabet also ifscript of national language is other than Roman);IVa) Date of birth;V) Address of holder if desired by the State;VI) Nationality of holder;VII) Signature of holder;VIII) Authority and, where necessary, conditionsunder which the licence is issued;IX) Certification concerning validity and authorizationfor holder to exercise privileges appropriate to licence;X) Signature of officer issuing the licence and the dateof such issue;XI) Seal or stamp of authority issuing the licence;XII) Ratings, e.g. category, class, type of aircraft,airframe, aerodrome control, etc.;XIII) Remarks, i.e. special endorsements relating tolimitations and endorsements for privileges, includingfrom 5 March 2008 an endorsement of languageproficiency, and other information required inpursuance to Article 39 of the Chicago Convention;XIV) Any other details desired by the State issuing thelicence.A20 I 2 A1-1.2.9.4 As of 5 March 2008, aeroplane, airship, helicopterand powered-lift pilots, air traffic controllers andaeronautical station operators shall demonstrate theability to speak and understand the language used forradiotelephony communications to the level specifiedin the language proficiency requirements in Appendix1.No endorsement of the requiredEnglish language proficiencyand / or level lower than Level 4(but corrective action plan filedby the licensing State to ICAO).Instructions forcompleting the detaileddescriptionA20-05 Indicate theassignment of theinvolved pilot (captain,co-pilot) and / or ELPlevel, if available<strong>SAFA</strong> Coordination Section Page 94 of 217© European Aviation Safety Agency, 2012 V 2.0
