Creating Initiation and Association Forms 303<br />

SET UP Using SharePoint Designer, open the SharePoint site where you created the<br />

SPD SBS Job Application workflow. Display the workflow settings for the workflow.<br />

Initiation Form<br />

Parameters<br />

1. On the Workflow Settings tab, click Initiation Form Parameters in the Variables<br />

group.<br />

The Association And Initiation Form Parameters dialog box opens.<br />

2. Click Add. The Add Field dialog box opens.<br />

3. In the Field name box, type Applicant Name, and in the Description box, type<br />

Please type the name of the person who has applied for the job.<br />

4. Leave the default values in the Information type and Collect from parameter<br />

during lists; that is, Single line of text and Initiation (starting the workflow).<br />

Note When using the Association And Initiation Form Parameters dialog box for a list<br />

or site workflow, the Add Field dialog box does not display the Collect From Parameter<br />

During list. For list and site workflows, only initiation forms are created, so any field you<br />

add will be an initiation form parameter.<br />

5. Click Next. The Column Settings dialog box is displayed.<br />

6. Leave the Default value box empty, and click Finish.<br />

Warning If you want the workflow to start automatically instead of manually, you must<br />

provide default values for all the initiation parameters. Otherwise, a warning dialog box<br />

is displayed when you publish the workflow.
