Most of the patients who had a stroke suffered a SRBD or
OSAHS, which occurred in the second half of the night, when
REM sleep preponderates.1,4,5 The same is true for infarct of
myocardial - patients with OSAHS have 500% more chance to
have myocardial infarct than people without it.1 Our patient is a
good example of the complications a patient with OSAHS can
have since he already had a stroke and infarct of myocardial.
Treatment in the OSAHS varies depending upon the primary
cause of the respiratory disturbance, but can range from
behavioral techniques, weight loss, the use of mechanical devices
to free the upper airway obstruction up to surgery. Our patient
will be using the CPAP since he is doing well with it and his cardiologist
wants him to avoid surgery.
Resumo
Não é raro ver pacientes queixando-se aos psiquiatras de
depressão, insônia, apatia, falta de motivação, hipersonia e que não
respondem aos antidepressivos (AD). Muitas vezes a síndrome da
apnéia hipopnéia obstrutiva do sono (SAHOS) com acordares relacionados
a esforços respiratórios (ARER) não é considerada no
diagnóstico diferencial ou como um segundo diagnóstico, o que
pode levar a frustração de ambos paciente e médico. SAHOS não é
condição rara, afetando 4% a 6% da população geral. No Brasil seis
a oito milhões de pessoas sofrem de SAHOS, a grande maioria não
diagnosticada ou diagnosticada e tratada incorretamente. SAHOS
está também associada com infarto do miocárdio, acidente vascular
cerebral, hipertensão, risco sete vezes maior de acidentes automobilísticos
e representa um fardo social. O objetivo da apresentação
deste caso é o de chamar a atenção para a SAHOS na população
psiquiátrica por sua tendência a obesidade pelo uso de neurolépticos
e AD, e pelo uso freqüente de benzodiazepínicos, que são con-
tra-indicadas nos pacientes com SAHOS por relaxarem os músculos
durante o sono e piorarem o quadro. Apresentamos aqui o caso
clínico de paciente de 56 anos de idade com SAHOS, que já apresentou
derrame, infarto do miocárdio e sofria de sonolência excessiva
durante o dia, falta de motivação, apatia, depressão, insônia inicial
e dificuldades de memória.
