Sialendoskopie bei obstruierenden Erkrankungen der großen Kopf-H<strong>als</strong>-<br />
Speicheldrüsen<br />

Fragestellung: Die Sialendoskopie stellt ein relativ neues minimal-invasives Verfahren zur<br />
Diagnostik und Behandlung obstruktiver Erkrankungen der großen Speicheldrüsen dar.<br />
Methode: In dieser retrospektiven Analyse berichten wir über unsere Erfahrungen bei 120<br />
Fällen von November 2006 bis Mai <strong>2011</strong> an der Grazer HNO-Universitätsklinik.<br />
Ergebnisse: Im Zeitraum 11/ 2006 bis 5/ <strong>2011</strong> führten wir 137 Sialendoskopien bei 120<br />
Patienten, davon 75 bei Erkrankungen der Glandula submandibularis und 62 der Glandula<br />
parotis. Dabei konnten 56 Speichelsteine entfernt (47x in der Glandula submandibularis, 9x<br />
in der Glandula parotis), 31 Gangstenosen dargestellt, 26x bougiert werden und 35x erfolgte<br />
die Korticoidinstillation. In 14 Fällen gelang die Steinentfernung nicht.<br />
80,95% aller Submandibularis-Patienten waren nach der Sialendoskopie dauerhaft<br />
beschwerdefrei, bei weiteren 15,88% kam es zur Besserung der Beschwerden.<br />
Für Parotissialendoskopien lauten die Zahlen 41,31% bzw. 32,60%.<br />
Diskussion: Die Sialendoskopie <strong>als</strong> minimal-invasive Methode ermöglicht eine effektive<br />
Diagnosestellung und in vielen Fällen die Behandlung obstruktiver<br />
Speicheldrüsenerkrankungen. In den meisten Fällen ist sie ambulant in Lokalanästhesie<br />
durchführbar. Durch Sialendoskopie konnte in 15 von 39 Fällen (38,46%) die Exstirpation der<br />
Glandula submandibularis, in welchen diese nach den bisher bei uns geltenden Indikationen<br />
erfolgt wäre, vermieden werden.<br />

Volumetric changes of the anterolateral thigh free flap following adjuvant<br />
radiotherapy in total parotidectomy reconstruction<br />

Objective: Still to date there are no data available showing the effect of radiation therapy<br />
(RT) on the volume of free flaps in patients after total parotidectomy and/or lateral skull base<br />
resections. The aim of this study was to determine the volume of anterolateral thigh free flaps<br />
pre- and post radiotherapy in a prospective set up.<br />
Methods: Seven patients with advanced squamous cell carcinoma of the parotid gland<br />
underwent anterolateral thigh free flap reconstruction following total parotidectomy/lateral<br />
skull base resections and neck dissections. Besides closing the surgical defect indication for<br />
reconstruction was face contouring and cosmesis. Intraoperative tumor and flap volume data<br />
were collected prospectively using the water displacement method. Before starting<br />
radiotherapy routine planning scans were done on all patients. All patients received<br />
postoperative radiotherapy (median radiation dose 55Gy). A minimum of 6 months after<br />
surgery CT of the head and neck was carried out and volumetric assessment was performed.<br />
Results: Six men and one woman with a mean age of 74 (range 41-89) years were included.<br />
