Conclusions: The anatomical and surgical experience of the surgeon assures the increased<br />

Rise of the ladette: has the epidemiology of nasal fractures in females in the<br />
UK changed?<br />

Introduction: According to several government reports, antisocial behaviour and violence,<br />
usually fuelled by alcohol, is on the rise in young females aged 16 to 20. The phenomenon,<br />
first noticed in 2001, has been coined ‘ladette culture’. The aim of this study is to determine<br />
whether there has been a change in the epidemiology of nasal fractures occurring in female<br />
populations.<br />
Methods: Multi-centred study across three hospit<strong>als</strong>. Retrospective operating theatre data<br />
for all females undergoing manipulation under anaesthesia (MUA) of nasal fractures over an<br />
8-year period from 2002 to 2009 were analysed. Case notes of all females undergoing MUA<br />
of nasal fractures over a 5 year period from 2004 to 2009 were <strong>als</strong>o reviewed retrospectively<br />
and the cited cause of fracture was noted.<br />
Results: 825% increase in nasal fractures in women aged 13-20 between 2002-2009.<br />
Conclusion: There is an increase in the number of women sustaining nasal fractures and<br />
this could be attributed to the rise in ‘ladette culture’.<br />

Erfahrungen mit individuell angepassten Septumobturatoren<br />

Hintergrund: In den vergangenen Jahren haben individuell angefertigte Obturatoren eine<br />
verstärkte Aufmerksamkeit erfahren. Bislang fehlen jedoch Erfahrungsberichte an einer<br />
größeren Patientenzahl unter Berücksichtigung der Beurteilung durch die<br />
Patienten.<br />
Methoden: Bei 64 Patienten, die zw. 2001 und 2009 mit einem individuell angefertigten<br />
Obturator versorgt worden waren, wurden die ätiologischen Aspekte der Perforation erfasst.<br />
Die Patienten wurden tel. hinsichtlich Ihrer Gesamtbeurteilung und der Beeinflussung<br />
einzelner Beschwerden durch die Obturatorversorgung befragt.<br />
Ergebnisse: Ein op. Eingriff am Septum wurde von 66% und ein Trauma von 16% der<br />
Patienten bei Erstvorstellung angegeben. Bei 11% der Patienten war ein Mb. Wegener<br />
gesichert. Jeweils 3% führten ihre Perforation auf digitales Manipulieren bzw.<br />
vasokonstriktorischen Nasensprayabusus zurück. Einmal blieb die Perforationsätiologie<br />
unbekannt. Auf einer Skala von 1 bis 10 gaben 65% der Patienten eine hohe Zufriedenheit<br />
nach Obturatorversorgung an. Bei allen befragten Symptomen konnte durch einen individuell<br />
angepassten Septumobturator eine statistisch signifikante Verbesserung erzielt werden. Am<br />
stärksten wurde das Symptom der Nasenatmungsbehinderung verbessert, gefolgt von<br />
Epistaxis, Schmerzen, Pfeifgeräuschen und Krustenbildung.<br />
