Methods: Nine patients who underwent ACL<br />
reconstruction with autogenous bone-patellar<br />
tendon-bone grafts using an all-inside technique<br />
were followed-up after a mean of 29 months<br />
(range 24-65). The mean age at the time of<br />
evaluation was 28 years (range 13-45). Clinical<br />
results were evaluated using the international<br />
knee documenttation committee (IKDC) and<br />
Lysholm score. A postoperative MRI was<br />
performed in all patients to evaluate the graft.<br />
Results: Using the IKDC evaluation system, 67% of<br />
patients were classified as nearly normal knees<br />
and 33% as abnormal knees. The pivot shift was<br />
positive in 33% of patients. Fifty-five percent of<br />
patients had a KT-1000 side-to-side difference of<br />
more than 3 mm. The mean Lysholm score was 93<br />
points. In the postoperative MRI the graft was<br />
continuous and with a homogeneous signal in<br />
22% of patients.<br />
Conclusion: All-inside technique offers several<br />
distinct advantages such as fixation at the original<br />
ACL insertion, tibial tunnel with no damage to the<br />
anterior cortical of the tibia and the tibial physis<br />
might not be violated in immature patients. In<br />
spite of the advantages described of this<br />
technique we consider that this procedure was<br />
technically demanding therefore limiting its use.<br />
Initial results are not encoraging enough to justify<br />
its practice in a habitual way. To our knowledge<br />
this is the first report of a series of patients<br />
evaluated with this technique.<br />
It is indicated only for strictly selected cases and<br />
must performed by surgeons with experience.<br />

Simultaneous High Tibial Osteotomy and ACL<br />

Purpose: The purpose of this study was to<br />
evaluate a group of active patients, with<br />
symptomatic knee instability and varus<br />
malalignment with medial unicompartmental<br />
arthrosis, wich underwent a simultaneous high<br />
tibial osteotomy and ACL reconstruction.<br />
Methods: Twenty patients with an average age of<br />
40 years (range 29-51) underwent arthroscopic<br />
ACL reconstruction and high tibial osteotomy in<br />
one surgical step. In all patients a lateral closing<br />
wedge and internally fixed with plate and screws<br />
was performed. Eighteen patients with a<br />
minimum two year follow-up were radiographic<br />
and clinically reviewed. Clinical evaluation was<br />
performed according to Lysholm score, Hospital<br />
for special surgery (HSS) and IKDC.<br />
Results: Mean anatomic tibiofemoral axis was<br />
corrected from 8º of varus preoperatively to 4º of<br />
valgus at final control. Complete range of motion<br />
was obtained in all patients, however one patient<br />
required mobilization under anesthesia. Mean<br />
Lysholm score was 94 points; mean HSS 91 and<br />
IKDC evaluation showed 5 A result, 10 B results<br />
and 3 C results. Mean manual maximum KT-1000<br />
difference to the opposite site was less than 3 mm<br />
in 12 patients, 4mm in 5 and the remaining was<br />
not evaluated due to bilateral pathology.<br />
Discussion: Indication for high tibial osteotomy<br />
and an ACL reconstruction in young active<br />
patients with varus malalignment and<br />
symptomatic instability is controversial. A<br />
simultaneous technique is very demanding and<br />
requires precise preoperative planning and<br />
surgical timing. These results showed that most<br />
patients could expect symptomatic improvement<br />
and return to recreational activities.<br />
