y IKDC guideline. Bone tunnels enlargement of<br />
more than 1 mm was identified in 5 femoral<br />
tunnels (5%) and 4 tibial tunnels (7%). One<br />
patients suffered from re-rupture and received<br />
revision operation.<br />
Conclusions: Periosteum has the potential to<br />
enhance tendon-bone healing. It can be expected<br />
to seal the intra-articular tunnel opening in an<br />
early period to avoid synovial fluid influx and<br />
undesirable tendon motion in the tunnel between<br />
the tendon and the bone. Femoral and tibial<br />
tunnel enlargement could be effectively reduced.<br />
Periosteum can be easily harvested at the<br />
proximal tibia from a routine incision for<br />
hamstring tendons harvesting. Satisfactory result<br />
can be achieved with the periosteum-enveloping<br />
hamstring tendon graft in ACL reconstruction.<br />

INTRODUCTION: PCL(posterior cruciate ligament)<br />
injuries have the potential for intrinsic healing<br />
and it is known that functional or clinical status<br />
could not be correlated with degree of clinical<br />
laxity. The goal of our study was to investigate the<br />
outcome of conservative treatment of the acute<br />
isolated PCL injury using cylinder cast<br />
immobilization with tibial supporter.<br />
MATERIALS AND METHODS: The subjects were<br />
10 acute isolate PCL injuries, with more than 2<br />
years of follow- up from January 1998 to July 2001.<br />
There were 9 male and 1 female patients, with an<br />
average age of 23(12-47) years (the epiphyseal<br />
plate was not completely closed in 3 patients).<br />
Originally there were 14 patients, but some<br />
patients were excluded by combined injuries and<br />
operations at other hospital. Diagnosis of the<br />
isolated PCL injury was done by physical<br />
examination and stress push radiography using<br />
Telos® device, KT 2000 maximal manual test and<br />
MRI. We applied a long leg splint or cylinder cast<br />
immobilization with tibial supporter with full<br />
extension of the knee for 6 weeks and then<br />
another six weeks of PCL brace with tibial<br />
supporter and posterior elastic rubber band to<br />
prevent posterior sagging of the proximal tibia.<br />
During treatment, we recommended quadriceps<br />
setting exercise and straight leg raising exercise.<br />
Contact sports were allowed 6 months after<br />
trauma.<br />
RESULTS: There were 7.5mm (4-10mm) side to<br />
side difference at initial stress film, and 3.4 mm(1-<br />
8 mm) at last follow-up. 4 patients had A, 6 had B<br />
in IKDC objective score and 88.2(63-94) points in<br />
IKDC subjective score, 95.4(86-100) points in OAK<br />
score at last follow-up.<br />
CONCLUSIONS: Acute isolated PCL injury treated<br />
with cylinder cast immobilization and PCL brace<br />
with tibial supporter to prevent posterior sagging,<br />
improved stability but some residual laxity<br />
remained. However, most of the patients were<br />
satisfied and the scores were excellent. The<br />
conservative treatment with cylinder cast<br />
immobilization and brace may be one of the<br />
recommendable treatment methods in isolated<br />
acute PCL injury.<br />

Osteomyelitis of the patella is a very rare<br />
condition and it is generally considered as a<br />
disease of childhood. The diagnosis is frequently<br />
delayed because of its rarity and variability in<br />
presentation. Authors report two cases of patellar<br />
osteomyelitis which was presented as prepatellar<br />
bursitis in children.<br />
