angle was measured on axial views at 30 .We<br />

decided this surgical procedure in case of severe<br />

trochlear dysplasia or obvious abnormal tracking<br />

of the patella.We performed the trochleoplasty<br />

with special ancillary to preserve the cartilage in<br />

four steps:<br />

We did<br />

- a clearance of the subchondral region of<br />

the groove starting above the groove<br />

- a division of the cartilage at the midline<br />

- a progressive deepthening<br />

- and fixation with two millimetric staples<br />

embedded in the cartilage to recreate a groove.<br />

Results<br />

No serious complication was observed.<br />

The trochlear angle decreased from an average<br />

pre-operative value at 151 to a post-operative<br />

average value at 133 . No recurrence of patellar<br />

dislocation was noted at the last follow-up.<br />

Detailled clinical results will be reported.<br />

Discussion<br />

In severe trochlear dysplasia, the presence of a<br />

high bump and an insufficient trochlear depth are<br />

responsible for patellar instability, and their<br />

persistance may limit the efficiency of the others<br />

surgical procedures.<br />

Conclusion<br />

Trochleoplasty is an option either in case of<br />

severe trochlear dysplasia with a proeminent<br />

bump or in case of recurrent dislocation after<br />

surgery. Nevertheless, we are still concerned to<br />

propose this operation in young population. A<br />

longer follow-up is needing.<br />

Backround: The elevation of the anterior tibial<br />

tubercle,although it has been first described 40<br />

years ago,remains controversial both on a clinical<br />

and on a bio-mechanical basis.It still offers a<br />

solution to the problem of chronic patellofemoral<br />

pain for young patients in their most productive<br />

years,who fail to respond to concervative<br />

treatment.<br />

Objective: This study aims to present the mid to<br />

long term results of a modification of the original<br />

Maquet procedure of a selected patient group.<br />

Materials & Methods: Retrospective review of preand<br />

peri- operative findings of 11 patients (7<br />

males),aged 28-61 years,treated by elevation of<br />

the tibial tubercle between 1993 and 2002.In all<br />

the cases the indication was persistent anterior<br />

knee pain that has not responded to concervative<br />

measures,excluding cases with proven<br />

osteoarthrosis of the other knee<br />

compartments.The operation consisted of an 1,3<br />

to 1,9cm elevation of a short tibial shingle.At a<br />

median follow up period of 5 years we evaluated<br />

the post-operative results by using subjective<br />

criteria(Cox's,Schmid's),a pain questionnaire (GR-<br />

SF/MPQ),the Oxford Knee score and objective<br />

criteria (Fulkerson's,Kujala's functional knee<br />

scores).<br />

Results: All but one reported excellent results<br />

subjectively.We found a statistically significant<br />

improvement of the post-operative GR-McGill<br />

score and of the Oxford Knee score.An ''excellent''<br />

median Fulkerson score of 92 and a ''good''<br />

median Kujala score of 87 were found.No<br />

significant short and long term complications<br />

were noticed.Ten of the patients report difficult in<br />

kneeling.<br />

Conclusions: The modified Maquet technique<br />

seems to be an effective operative procedure only<br />

for the treatment of symptomatic patellofemoral<br />

osteoarthrosis after concervative means have<br />

failed.A strict patient selection process must be<br />

used.The complication rate can be limited by<br />

strict adherence to the details of the surgical<br />

technique.<br />
