Acknowledgements A mapping exercise of this magnitude required the input of many persons and it was a team effort. Firstly I would like to thank the men and women who we interviewed and who opened up and shared their personal stories and experiences with us. It may not have always been easy to give such details but they accommodated us and without their input, the project would not have been possible.
