APPENDIX N - EMERGENCY PROTOCOL CARD<br />

Each first aid kit should have an Emergency Protocol Card <strong>for</strong> respondents to use <strong>in</strong> deal<strong>in</strong>g with an<br />

emergency situation. It should also be posted by all phones near the area of activity. This <strong>in</strong><strong>for</strong>mation<br />

should be conveyed to emergency personnel over the phone if an accident occurs:<br />

1. Dial the Emergency Phone Number, e.g., 911 or _________________________<br />

2. Give the specific address of facility and location of access doors:_______________<br />
