Injury or Illness dur<strong>in</strong>g Competition Season<br />

Should your son/daughter/ward susta<strong>in</strong> an <strong>in</strong>jury or contract an illness requir<strong>in</strong>g medical attention dur<strong>in</strong>g<br />

the competitive season, notify the teacher/coach and complete the “Request to Resume Athletic<br />

Participation Form”.<br />

ELEMENTS OF RISK NOTICE<br />

The risk of <strong>in</strong>jury exists <strong>in</strong> every athletic activity. Falls, collisions and other <strong>in</strong>cidents may occur and<br />

cause <strong>in</strong>jury. Due to the very nature of some activities, <strong>in</strong>juries may range from m<strong>in</strong>or spra<strong>in</strong>s and stra<strong>in</strong>s<br />

to more serious <strong>in</strong>juries affect<strong>in</strong>g the head, neck or back. Some <strong>in</strong>juries can lead to paralysis or prove to<br />

be life threaten<strong>in</strong>g. These <strong>in</strong>juries result from the nature of the activity and can occur without fault on<br />

either the part of the student athlete, or the school board or its employees or agents or the facility where<br />

the activity is tak<strong>in</strong>g place. By choos<strong>in</strong>g to participate <strong>in</strong> the activity, you are assum<strong>in</strong>g the risk of an<br />

<strong>in</strong>jury occurr<strong>in</strong>g. The chances of an <strong>in</strong>jury occurr<strong>in</strong>g can be reduced by carefully follow<strong>in</strong>g <strong>in</strong>structions<br />

at all times while engaged <strong>in</strong> the activity. The (name of School Board) attempts to manage as effectively<br />

as possible the risk <strong>in</strong>volved <strong>for</strong> student athletes while participat<strong>in</strong>g <strong>in</strong> school athletics. A copy of the<br />

Risk Management Policy <strong>for</strong> <strong>in</strong>terschool sports is available <strong>in</strong> the school should you require more<br />

<strong>in</strong><strong>for</strong>mation.<br />

STUDENT ATHLETE ACCIDENT INSURANCE NOTICE<br />

The (name of School Board) does not provide any accidental death, disability,<br />

dismemberment/medical/dental expenses <strong>in</strong>surance on behalf of the student athletes participat<strong>in</strong>g <strong>in</strong> the<br />

activity. For coverage of <strong>in</strong>juries, you are encouraged to consider the Student Accident Insurance Plan<br />

made available by the school to parents at the beg<strong>in</strong>n<strong>in</strong>g and throughout the school year.<br />
