• Health Insurance: As an Employee of the <strong>Hospital</strong> you are eligible<br />

<strong>to</strong> avail of the SVUH VHI Group Scheme (29/1832). The relevant<br />

subscription will be deducted from your salary payments. These<br />

details are sent from VHI <strong>to</strong> Salaries directly.<br />

• Additional Voluntary Contributions/Personal Retirement Savings<br />

Accounts: Please contact the Benefits Section, HR Department for<br />

further information.<br />

<strong>Hospital</strong> Employer Tax Number: 6358585i. This number may be<br />

required if Employees are registering for tax purposes.<br />

Employee Number: All Employees are provided with an Employee<br />

Number on commencement of employment. Employee Numbers must<br />

be used on all correspondence issued <strong>to</strong> HR or the Salaries Office.<br />

Form P60: The Salaries Office issues a Form p60 which provides details<br />

of Pay and Tax for the preceding calendar year at the end of January<br />

each year <strong>to</strong> all Employee on the payroll. This form is attached <strong>to</strong> the<br />

Employees Payslip. Employees are asked <strong>to</strong> note that this is an official<br />

document and must be retained.<br />

Hours of Work: Normal working hours exclusive of meal breaks are<br />

stated in your contract of employment.<br />

Additional Hours (Overtime/On Call): Because of the nature of the<br />

service provided by the <strong>Hospital</strong>, Employees are expected <strong>to</strong> be able<br />

<strong>to</strong> work additional hours as may be required by their Head of Service/<br />

Line Manager from time <strong>to</strong> time. In all cases the appropriate Head of<br />

Service / Line Manager must approve any overtime in advance.<br />

Overtime/on call rates are paid in accordance with Department of<br />

Health & Children regulations.<br />
