Section 11: Leaving the <strong>Hospital</strong><br />
Notice requirements: All Employees are expected <strong>to</strong> give the<br />
minimum notice stipulated in their Contract of Employment of their<br />
intention <strong>to</strong> resign from the <strong>Hospital</strong>.<br />
Notice requirements are as stated in your contract of employment.<br />
How do I resign? A resignation is a voluntary termination of<br />
employment by the Employee. Employees can resign by giving notice<br />
in writing <strong>to</strong> their Head of Service/Line Manager by completing the<br />
“<strong>St</strong>aff Termination Form”, which is located on the Intranet or available<br />
from the HR Department.<br />
Exit Questionnaire: Employees will be asked <strong>to</strong> complete an Exit<br />
Questionnaire when resigning <strong>to</strong> provide feed<strong>back</strong> on their career at<br />
the <strong>Hospital</strong>.<br />
We recognise the importance of understanding why Employees leave<br />
the <strong>Hospital</strong> and the purpose of the Exit Questionnaire is <strong>to</strong> obtain<br />
feed<strong>back</strong> on working conditions and experiences within the <strong>Hospital</strong>.<br />
All information is confidential. We will use the information gathered <strong>to</strong><br />
assess general employment trends and <strong>to</strong> make improvements in the<br />
workplace where possible, as we strive <strong>to</strong> continuously improve<br />
Employee satisfaction, staff retention and the service we provide <strong>to</strong><br />
our patients.<br />
