CONTENT<br />
1. Human factors contributing to accidents:<br />
– contradictory data;<br />
– disagreement between watch members;<br />
– available relevant information not<br />
evaluated;<br />
– course plan not followed;<br />
– distraction;<br />
– failure to follow communication<br />
protocol;<br />
– inadequate organization;<br />
– failure to monitor ship’s progress;<br />
– etc.<br />
2. Chain of errors and how to break it.<br />
SUGGESTED METHODS<br />
Candidates and instructor<br />
• Analyse human factors contributing to<br />
accidents. When a shipping accident<br />
occurs in Canadian waters, the Marine<br />
Investigation Branch of the Transportation<br />
Safety Board prepares a public report.<br />
Analysis of these reports makes it possible<br />
to target and extract some of the human<br />
factors directly or indirectly responsible<br />
for accidents. To be performed in the<br />
classroom, with the help of the instructor.<br />
• After analysing case studies, discuss<br />
preventive methods. Once the elements<br />
that contributed to the accident have been<br />
identified, examine what steps would have<br />
ensured safe navigation. This is performed<br />
in the classroom with the help of the<br />
instructor.<br />
