394 Section 4: Evidence-based classificationOfficial classifications, however, have never been fully consistent with the syndromesrecognized by clinicians and researchers, but the advent of DSM-III was asea-change that abandoned the traditional principles of psychopathologic clinicalinvestigation. The change seriously limited the diagnostic acumen of this generationof young psychiatrists worldwide.Much of what is detailed in this book is not incorporated in present classificationsand is not taught in the disciplines responsible for the care of the psychiatricallyill. These formulations, however, have face validity and are supported bymany studies. The mastery of it, however, is an on-going, difficult process.Considering the recommended changes in classification offered here and usingthe detailed psychopathology presented throughout the text in the diagnosis ofpatients are the beginnings of the effort.
