394 Section 4: Evidence-based classificationOfficial classifications, however, have never been fully consistent with the syndromesrecognized by clinicians <strong>and</strong> researchers, but the advent <strong>of</strong> DSM-III was asea-change that ab<strong>and</strong>oned the traditional principles <strong>of</strong> psychopathologic clinicalinvestigation. <strong>The</strong> change seriously limited the diagnostic acumen <strong>of</strong> this generation<strong>of</strong> young psychiatrists worldwide.Much <strong>of</strong> what is detailed in this book is not incorporated in present classifications<strong>and</strong> is not taught in the disciplines responsible for the care <strong>of</strong> the psychiatricallyill. <strong>The</strong>se formulations, however, have face validity <strong>and</strong> are supported bymany studies. <strong>The</strong> mastery <strong>of</strong> it, however, is an on-going, difficult process.Considering the recommended changes in classification <strong>of</strong>fered here <strong>and</strong> usingthe detailed psychopathology presented throughout the text in the diagnosis <strong>of</strong>patients are the beginnings <strong>of</strong> the effort.
