capable of only poor quality, which will need<br />

replacement much sooner. A poor-quality image is<br />

a false economy in abdominal scanning.<br />

Machine capabilities and functions<br />

The availability and ease of use of various functions<br />

differ from machine to machine. Some of the<br />

important issues to consider when buying a<br />

machine include:<br />

colour/power and spectral Doppler through all<br />

Doppler controls—ease of use, programmable<br />

Ergonomics<br />

Good ergonomics contribute considerably to the<br />

success of the service provided. The machine must<br />

be usable by various operators in all the required situations.<br />

There is a significant risk of work-related<br />

musculoskeletal disorders (WRMSD) 2 if careful<br />

consideration is not given to the scanning environment<br />

(see p. 12). When choosing and setting up a<br />

scanning service, forethought should be given not<br />

only to the design of the ultrasound machine, but<br />

also to the seating arrangements and examination<br />

couch. These should all be adjustable in order to<br />

facilitate the best scanning position for the operator.<br />

Other considerations include:<br />

System dimensions and steering. The<br />

requirement for the system to be portable, for<br />

example for ward or theatre work, or mobile<br />

for transportation to remote clinics. Machines<br />

used regularly for mobile work should be<br />

robust and easy to move.<br />

Moveable (swivel and tilt) monitor and control<br />

panel, including height adjustment for different<br />

operators and situations.<br />

Keyboard design, to facilitate easy use of the<br />

required functions, without stretching or<br />

twisting.<br />

Hand-held portable machines are an option<br />

that may be considered.<br />

Maintenance issues<br />

It is useful to consider the reliability record of the<br />

chosen equipment, particularly if it is to operate in<br />

out-reach clinics, or without available backup in<br />

the case of breakdown. Contacting other users may<br />

prove useful.<br />

Various maintenance contract options and costs<br />

are available, including options on the replacement<br />

of probes, which should be taken into account<br />

when purchasing new equipment.<br />

Upgradeability<br />

A machine which is potentially upgradeable has a<br />

longer, more cost-effective life and will be supported<br />

by the manufacturer over a longer period of<br />

time. Consideration should be given to future software<br />

upgrades, possible effects and costs and other<br />

available options for the future, such as additional<br />

transducers or add-on Doppler facilities.<br />

Links to image-recording devices<br />

Most ultrasound machines are able to link up to<br />

most types of imaging facility, whether it be a simple<br />

black and white printer or a radiology-wide<br />

photographic archiving and communications (PAC)<br />

system. There may be costs involved, however, in<br />

linking your new machine to your preferred imaging<br />

device.
