individual or LAR requests a CLASS Program service that the CMA or DSA [begin addition]

[begin addition] (A) does not meet the criteria described in §45.214(b) of this division;
(B) does not meet the requirements described in Subchapter F of this chapter (relating to
Adaptive Aids and Minor Home Modifications); or
(C) if the service is transition assistance services, exceeds the service limit described in
§45.218(c) of this division.
(2) If the CMA is required to send a written notice of denial of a CLASS Program service
as described in paragraph (1) of this subsection, the CMA must:

described in subsection (d)(2) of this section, submit to DADS for its review:

renewal IPC or proposed revised IPC, which includes the type and amount of CLASS Program
services in dispute and not in dispute, and is signed and dated by:

manager; and

[begin addition] (II) [end addition] [begin deletion] (ii) [end deletion] the case
[begin addition] (III) [end addition] [begin deletion] (iii) [end deletion] the DSA;
[begin addition] (ii) [end addition] [begin deletion] (B) [end deletion] the IPPs; and
[begin addition] (iii) [end addition] [begin deletion] (C) [end deletion] the new habilitation
plan or any revised habilitation plan; and
[begin addition] (B) [end addition] [begin deletion] (3) [end deletion] send a copy of the
proposed renewal or proposed revised IPC to the CDSA, if applicable.
[begin addition] (3) If the individual or LAR requests a service described in §45.218(b) of
this division in an amount that exceeds the service limit for that service, the CMA must:
(A) in accordance with the time frames described in subsection (d)(2) of this section, submit
to DADS for its review:
(i) the proposed renewal IPC or proposed revised IPC that includes the type and amount of
CLASS Program services in dispute and not in dispute and is signed and dated by:
