[begin addition] (i) [end addition] a licensed physician;[,]<br />

[begin addition] (ii) an RN [end addition] [begin deletion] nurse, [end deletion]<br />

[begin addition] (iii) an [end addition] occupational therapist;[,]<br />

[begin addition] (iv) [end addition] physical therapist;[begin deletion] , or [end deletion]<br />

[begin addition] (v) a [end addition] speech [begin addition], hearing, <strong>and</strong> language therapist;<br />

or [end addition]<br />

[begin addition] (vi) for computer assistive technology, augmentative communication devices,<br />

or environmental controls, another appropriate professional, including a rehabilitation engineer;<br />

<strong>and</strong> [end addition] [begin deletion] pathologist. Other appropriate professionals, including<br />

rehabilitation engineers, may provide assessments <strong>and</strong> specifications for computer assistive<br />

technology, augmentative communication devices, or environmental controls. Providing<br />

assessments <strong>and</strong> obtaining written specifications is reimbursable as a nursing service if<br />

performed by a nurse <strong>and</strong> as a therapy service if performed by an occupational therapist, physical<br />

therapist, or speech pathologist <strong>and</strong> authorized on the individual service plan. If another<br />

appropriate professional is required to provide the assessment <strong>and</strong> develop the specifications for<br />

computer assistive technology, environmental controls, or augmentative communication devices,<br />

the actual cost of writing the specifications <strong>and</strong> the follow-up orientation/training visit is<br />

reimbursed as part of the invoice cost up to a maximum of $500. The provider must maintain<br />

documentation to support the cost of developing the specifications; [end deletion]<br />

[begin addition] (B) obtain comparative price quotes or use a price list to document prices of<br />

the adaptive aid from a minimum of three suppliers or annually select a supplier based on the<br />

lowest prices from the quotes or price list for the main types of adaptive aids purchased by the<br />

HCSSA; <strong>and</strong><br />

(C) document in the vendor records;<br />

(i) the names of the suppliers from whom all quotes or price lists were obtained:<br />

(ii) the amount of the quotes or price lists;<br />

(iii) the items for which the quotes or price lists were requested; <strong>and</strong><br />

(iv) the dates the quotes or price lists were obtained.<br />

(2) A HCSSA may claim reimbursement for an assessment for an adaptive aid:<br />
