[(B) the individual's IPP as described in paragraph (1)(B) of this subsection.[end deletion]<br />

(b) [begin addition] The DSA must:<br />

(1) within 30 calendar days after receipt of the partially completed form, obtain the<br />

specifications regarding the minor home modification in accordance with §45.614 of this<br />

division (relating to Requirements for Specifications for a Minor Home Modification) <strong>and</strong> ensure<br />

that Part D of the Adaptive Aids, Medical Supplies <strong>and</strong> Minor Home Modifications form is<br />

completed; <strong>and</strong><br />

(2) within 60 calendar days after obtaining the specifications:<br />

(A) if the minor home modification costs more than $1000, obtain bids from vendors in<br />

accordance with §45.615 of this division (relating to Bid Requirements for Minor Home<br />

Modification);<br />

(B) select a vendor to complete construction of the minor home modification; <strong>and</strong><br />

(C) complete Part C of the Adaptive Aids, Medical Supplies <strong>and</strong> Minor Home Modifications<br />

form <strong>and</strong> send the form to the CMA; <strong>and</strong><br />

(4) before construction of the minor home modification:<br />

(A) obtain written approval for construction of the modification from the owner of the<br />

property in question, unless such approval is granted in an applicable lease agreement; <strong>and</strong><br />

(B) ensure that the selected vendor obtains any required building permits. [end addition]<br />

[begin deletion] DADS reviews the documentation described in subsection (a)(2) of this section<br />

<strong>and</strong> determines whether the proposed IPC is authorized in accordance with §45.216 of this<br />

chapter (relating to DADS' Review of an Enrollment IPC) or §45.223 of this chapter (relating to<br />

Renewal <strong>and</strong> Revision of an IPC). [end deletion]<br />

(c) [begin addition] A CMA must, within 14 calendar days after receipt of the form described<br />

in subsection (b)(2)(C) of this section:<br />

(1) complete Part D of the Adaptive Aids, Medical Supplies <strong>and</strong> Minor Home<br />

Modifications form, evidencing that the criteria described in §45.214(b) of this chapter (relating<br />

to Development of Enrollment IPC) are met; [end addition] [begin deletion] DADS notifies a<br />

DSA, in the electronic billing system, of whether the proposed IPC is authorized. DADS notifies<br />

a CMA, in writing, of whether the proposed IPC is authorized.]<br />

[(d) If DADS authorizes the proposed IPC for payment of the specifications, the DSA must:]<br />

[(1) within 30 calendar days after the date DADS authorizes the IPC, obtain the<br />

specifications regarding the requested minor home modification in accordance with §45.614 of<br />

this division;]<br />
