services, but the documentation of services delivered form, or facsimile, is missing for the period
for which services are reimbursed. [begin addition] DADS [end addition] [begin deletion] DHS
[end deletion] applies the error to the total number of units documented on the time sheet.
(2) The attendant, [begin addition] LVN, RN, [end addition] [begin deletion] nurse, or [end
deletion] [begin addition] occupational therapist, physical therapist, or speech, hearing, and
language [end addition] therapist leaves the entire record of time section blank. [begin addition]
DADS [end addition] [begin deletion] DHS [end deletion] applies the error to the total number of
units documented on the time sheet.
(3) [begin addition] DADS [end addition] [begin deletion] DHS [end deletion] reimburses
the [begin addition] CBA [end addition] provider [begin deletion] agency] for hours that exceed
the authorization given by [begin addition] DADS [end addition] [begin deletion] DHS [end
deletion]. [begin addition] DADS [end addition] [begin deletion] DHS [end deletion] applies the
error to the total number of [begin addition] hours [end addition] [begin deletion] units [end
deletion] reimbursed in excess of the [begin addition] number of hours [end addition] [units]
authorized by [begin addition] DADS [end addition] [begin deletion] DHS [end deletion], unless
purchased following emergency procedures. For nursing tasks, the maximum monthly hours that
may be reimbursed is the number of hours listed under "NURSING TASKS, Direct Nursing
Performed by [begin addition] HCSSA [end addition] [begin deletion] HCSS Provider [end
deletion]" on the [begin addition] nursing service plan [end addition] [begin deletion] individual
service plan/nursing service plan [end deletion].
(4) [begin addition] DADS [end addition] [begin deletion] DHS [end deletion] reimburses
the [begin addition] CBA [end addition] provider [begin deletion] agency [end deletion] for any
waiver service that is not identified on the [begin addition] ISP [end addition] [begin deletion]
individual service plan [end deletion] and attachments, unless the service was provided as a
result of an emergency and is supported by backup documentation supplied [begin addition] by
the CBA provider [end addition] within seven [begin addition] DADS workdays after [end
addition] [begin deletion] DHS work days from [end deletion] the date the emergency was
determined. [begin addition] DADS [end addition] [begin deletion] DHS [end deletion] applies
the error to the entire amount reimbursed for such services.
(5) [begin addition] DADS [end addition] [begin deletion] DHS [end deletion] reimburses
the [begin addition] CBA [end addition] provider [begin deletion] agency [end deletion] for
hours that exceed the total number of hours recorded on the documentation of services delivered
form or facsimile. [begin addition] DADS [end addition] [begin deletion] DHS [end deletion]
applies the error to the total number of [begin addition] hours [end addition] [begin deletion]
units [end deletion] reimbursed in excess of the [begin addition] hours [end addition] [begin
deletion] units [end deletion] recorded on the time sheet. If the sum of the daily total of hours
does not equal what is written in the monthly total blank, the lesser of the two totals is used to
calculate the total number of hours subject to the error.
(6) [begin addition] DADS [end addition] [begin deletion] DHS [end deletion] reimburses
the [begin addition] HCSSA [end addition] [begin deletion] provider agency [end deletion] for
