(B) the individual's IPP; <strong>and</strong><br />

requirement described in paragraph [begin addition] (2) [end addition] [begin addition] (1) [end<br />

deletion] of this subsection, submit to DADS:<br />

(A) the completed Request for Adaptive Aids, Medical Supplies, <strong>and</strong> Minor Home<br />

Modifications form [begin deletion] as required by subsection (f) of this section [end deletion];<br />

(B) the proposed enrollment IPC, proposed renewal IPC, or proposed revised IPC as<br />

described in paragraph [begin addition] (2) [end addition] [begin deletion] (1) [end deletion] (A)<br />

of this subsection, as applicable;<br />

(C) the individual's IPP as described in paragraph [begin addition] (2) [end addition] [begin<br />

deletion] (1) [end deletion] (B) of this subsection; <strong>and</strong><br />

(D) documentation regarding bids as required by §45.606 of this division.<br />

[begin addition] (d) [end addition] [begin deletion] (h) [end deletion] DADS reviews the<br />

documentation described in subsection [begin addition] (c)(3) [end addition] [begin deletion]<br />

(g)(2) [end deletion] of this section <strong>and</strong> determines whether the proposed IPC is authorized in<br />

accordance with §45.216 of this chapter or §45.223 of this chapter.<br />

[begin addition] (e) [end addition] [begin deletion] (i) [end deletion] DADS notifies a DSA, in<br />

the electronic billing system, of whether the proposed IPC is authorized. DADS notifies a CMA,<br />

in writing, of whether the proposed IPC is authorized.<br />

§45.605. Requirements for Specifications for an Adaptive Aid.<br />

(a) [begin addition] For [end addition] [begin deletion] If DADS authorizes payment for<br />

specificationsfor [end deletion] an adaptive aid costing $500 or more [begin deletion] in<br />

accordance with §45.604(c) of this division (relating to Requirements For Authorization to<br />

Purchase an Adaptive Aid Costing $500 or More) [end deletion], a DSA must:<br />

(1) obtain [begin deletion] the [end deletion] specifications from a licensed professional<br />

required by DADS for that adaptive aid as described in the CLASS Provider Manual; <strong>and</strong><br />

(2) ensure that the specifications:<br />

(A) include a complete description of the adaptive aid; <strong>and</strong><br />

(B) are approved, in writing, by the individual or LAR <strong>and</strong> the DSA by completing the<br />

Specifications for Adaptive Aids or Minor Home Modifications form as described in the CLASS<br />

Provider Manual.<br />
