TERAPIA CU FUMARAŢI ÎN SCLEROZA MULTIPLĂ Letiţia Palaghiţă-Banias 1 , Zoltan Major 2 , Cristina Cătană 3 , Tibor L Krausz 3 , Vl Şandor 3 Rezumat. Terapia actuală a sclerozei multiple este orientată pe DMD (disease modifying drugs) recomandate de Multiple Sclerosis Therapy Consensus Group (MSTCG). DMD necesită administrare parenterală, iar eficacitatea este parţială. Includerea unor noi compuşi chimici, cu administrare orală este binevenită în armamentariul terapeutic al sclerozei multiple. Fumaraţii constituie o astfel de clasă de medicamente. În scleroza multiplă, mecanismul autoimun implică în mod crucial limfocitele Th1. Date obţinute in vitro, pe modele experimentale de encefalită autoimună la şoarece şi in clinică în scleroza multiplă recurent-remisivă (RRMS- remitting-relapsing multiple sclerosis) demonstrează capacitatea derivaţilor de acid fumaric de a produce comutarea Th1-Th2. Fumaraţii au fost investigaţi în 2 studii clinice de faza II în RRMS, având ca end-point primar gadolinium-enhanced lesions (GdE). Fumaraţii reduc semnificativ numărul şi volumul GdE. Începând cu toamna anului 2007, se derulează 2 studii clinice de faza III, în Europa şi America de Nord, pentru evaluarea eficacităţii fumaraţilor în RRMS. Cuvinte cheie: fumaraţi, DMD, scleroza multiplă Introducere În etiopatogenia sclerozei multiple (MS-multiple sclerosis) intervenţia mecanismelor autoimune este esenţială. În iniţierea inflamaţiei, în demielinizare şi distrucţia axonală, intervin citokine proinflamatorii (IL-1, IL-6, IFN-γ, TNF-α), radicali liberi (NO) şi proteaze. La ora actuală un aport considerabil în terapia sclerozei multiple îl au medicamentele care modifică evoluţia bolii (DMD-disease modifiyng drugs), administrate conform recomandărilor MSTCG (Multiple Sclerosis Therapy Consensus Group)(3,4,5). Fumarate therapy in multiple sclerosis Actual treatment of multiple sclerosis is oriented towards disease modifying drugs (DMD) recommended by Multiple Sclerosis Therapy Consensus Group (MSTCG). DMD requires parenteral administration and has partial efficacy. Novel orally administered chemical compounds would be beneficial in multiple sclerosis treatment. Such class would be the fumarates. The autoimmune mechanisms in multiple sclerosis involves Th1 lymphocytes. In vitro data, in experimental autoimmune encephalitis and clinical remitting-relapsing multiple sclerosis (RRMS) demonstrates the ability of fumaric acid compounds in provoking Th1-Th2 shift. Fumarates were investigated in 2 phase II clinical trials in RMSS having primary endpoint gadolinium-enhanced lesions (GdE). Fumarates reduce significantly the volume and number of GdE. Since 2007 two phase-III trials are in course in Europe and North America regarding fumarate efficacy in RMMS. Introducerea în tratamentul sclerozei multiple a noi clase de medicamente, active pe cale orală este binevenită.(6,7,8,9). Derivaţii de acid fumaric au fost utilizaţi în tratamentul unor boli inflamatorii prin mecanism Th1, cum ar fi psoriazisul, cu rezultate remarcabile. Fumaraţii in vitro au efecte imunomodulatoare, neuroprotective şi detoxifiante (10,11). Studii clinice preliminare susţin utilizarea lor în tratamentul sclerozei multiple(12,13). Imunomodularea in vitro prin fumaraţi MHF inhibă producerea IFN-γ în celulele dendritice cultivate în vitro, dependent de 65
