2. Voyage Log. Include the dates <strong>and</strong> ports of arrival, vessel agent(s), last port <strong>and</strong> country of<br />

call, <strong>and</strong> next port <strong>and</strong> country of call (when known).<br />

3. If vessel owners/operators have violated any effluent limit, they must document the violation<br />

<strong>and</strong> record:<br />

� A description of the violation,<br />

� Date of the violation,<br />

� Name, title, <strong>and</strong> signature of the person who identified the violation,<br />

� Name, title, <strong>and</strong> signature of the person who is recording the violation (if different from<br />

person who identified the violation),<br />

� If a Corrective Action Assessment pursuant to Part 3.2 is needed, attach a copy or<br />

indicate where the corrective action assessment is stored, <strong>and</strong><br />

� If a Corrective Action Assessment was previously conducted pursuant to Part 3.2 (<strong>and</strong><br />

revisions are not needed for this violation of the effluent limit), a reference to that<br />

previous corrective action assessment.<br />

4. Log of findings <strong>and</strong> any deficiencies <strong>and</strong> problems identified during routine visual inspections<br />

<strong>and</strong> extended unmanned inspections, including a discussion of any corrective actions, if<br />

applicable. Include date, inspector’s name, findings, <strong>and</strong> corrective actions planned or taken. If<br />

no deficiencies or problems are found during a routine visual inspection, the vessel<br />

owner/operator shall record that the inspection was completed with the inspector’s name <strong>and</strong><br />

date. Routine visual inspections <strong>and</strong> extended unmanned inspections (if applicable) must be<br />

recorded as completed.<br />

5. Analytical results of all monitoring conducted, including sample documentation, results, <strong>and</strong><br />

laboratory quality assurance (QA) documentation. Log of findings from comprehensive annual<br />

vessel inspections conducted, including a discussion of any corrective actions planned or taken<br />

required. Include date, inspector’s name, findings, <strong>and</strong> a description of the corrective actions<br />

taken.<br />

6. Log of findings from comprehensive annual vessel inspections, including a discussion of any<br />

corrective actions planned or taken. Include date, inspector’s name, findings, <strong>and</strong> a description<br />

of the corrective actions taken.<br />

7. Log of findings from drydock inspections conducted including a discussion of any corrective<br />

actions planned or taken. Include date, inspector’s name, findings, <strong>and</strong> a description of the<br />

corrective actions taken.<br />

8. Record of any specific requirements given to your vessel by EPA or its authorized<br />

representative <strong>and</strong> how those requirements have been met.<br />

9. Additional maintenance <strong>and</strong> discharge information to be recorded <strong>and</strong> kept in a log on the<br />

vessel:<br />
