Settmacher (MD) Surgical Treatment Options for Liver Tumours Part 1: Entities, Diagnostics, Indications, Resection and Transplantation Techniques 1. Entities A distinction is made between benign and malignant liver tummours. Benign liver tumours incclude focal-nodular hyperplasia (FNH), haemangioma and liver cell adenoma. The adenoma represents a transitional form which may progress to beccome a malignant hepatic cell carcinoma. The haemangioma is the most common benign tumour of the liver (1). Malignant liver tumours are subdivided into primary and secondary lesions (metasttases). 90% of malignant liver tumours represent metastases of which the primary tumour is located to 85% in the colon and rectum and to 15% in other organs such as stomach, pancreas and ovaries. The remaining 10% of malignnant liver tumours comprise primary liver tumours. With a share of 80%, the hepatocelllular carcinoma (HCC) is the most common primary liver tumour, while the intrahepatic cholangiocellular carcinoma is the second most common primmary liver tumour with a share of 10-20% (2). Hybrids between HCC and CCC account for approx. 1% of all primary liver tumours. The incidence of HCC in men is 13.8 cases per 100,000 population per year; in women, however, it is only 0.8 cases per 100,000 population per year (3). The incidence of intrrahepatic CCC is 0.44 cases in men and 0.56 cases in women per 100,000 population per year (4). Extrahepatic bile duct tumours (Klatskin tumours – malignancies affecting the common bile duct) and gallbbladder tumours are rare. The incidence of extrahepatic bile duct cancer in men is 0.74 cases per 100,000 population per year, whereas in women it is merely 0.57 cases per 100,000 population per year (5). The incidence of gallbladdder carcinoma in women is 1.5 cases per 100,000 population per year, in men it is only 0.8 cases per 100,000 population per year (6). The figures above show the incidences in varioous European countries. It is particularly remarkable that the incidences for the various liver tumours differ in other parts of the globe. Primary liver tumours are rarely of mesencchymal origin as is the case with the primary sarcoma of the liver and haemangioendothelioma as well as haemangioendothelial sarcoma (7,8). اخليارات اجلراحية ملعاجلة اأورام الكبد اجلزء االأ ول:‏ ماهو هذا املرض،‏ الوسائل التشخيصية،‏ االإستئصال وتقنيات زرع الكبد 1. ماهو هذا املرض:‏ لقد مت التمييز ما بني اأورام الكبد السليمة واأروام الكبد اخلبيثة.‏ تتضمن اأورام الكبد الفرط التصنع املوضع العقدي )FNH( الورم الوعائي الدموي،‏ اأورام اخلاليا الكبدية الغدية.‏ اأن الورم الغدي ميثل ‏شكل انتقايل والذي ميكن اأن يتطور اإىل كارسبنوما اخلاليا الكبدية اخلبيثة.‏ يعترب الورم الوعائي الدموي اأكرث الأ ورام الكبدية السليمة .)1( تقسم اأورام الكبد اخلبيثة اإىل اأورام بدائية واأورام ثانوية ‏)انتقالية(.‏ %90 من اأورام الكبد اخلبيثة هي اأورام انتقالية والتي يتوضع الورم البدئي يف % 85 احلالت يف الكولون واملستقيم بينما % 15 يف الأ عضاء الأ خرى مثل املعدة،‏ البنكرياس واملبيضني.‏ تشكل اأورام الكبد البدئية ال %10 الباقية من اأورام الكبد اخلبيثة HCC حيث متثل كارسينوما اخللية الكبدية اأكرث اأورام الكبد البدئية ‏شيوعاً‏ )80 %( بينما تعترب كارسينوما الأ قنية الصفراوية )CCC( داخل الكبد ثاين ورم كبدي بدئي من حيث الشيوع والذي يساهم ‏)‏‎10‎‎20‎ %()2(. تواجد CCC و HCC معاً‏ بشكل %1 من جميع اأورام الكبد البدئية.‏ اإن معدل حدوث كارسينوما اخللية الكبدية يف الرجال 13،8 حالة لكل 100 األف نسمة بالسنة،‏ بينما معدل حدوثه لدى النساء 0،8 حالة لكل 100 األف نسمة بالسنة )3(. معدل حدوث كارسينوما الأ قنية الصفراوية داخل الكبد هو ,0 44 يف الرجال و‎0,56‎ من النساء لكل 100 األف نسمة من السكان)‏‎4‎‏(.‏ اأورام خارج الكبد لالأ قنية الصفراوية ‏)اأورام كالتسكني وهي الأ ورام اخلبيثة التي تصيب القناة الصفراوية اجلامعة(‏ واأورام املرارة اأورام نادرة.‏ اإن معدل حدوث ‏رسطان القناة الصفراوية خارج يف الرجال 0,74 يف كل 100 األف نسمة خالل السنة،‏ بينما يف النساء فقط 0,57 حالة 100 األف نسمة بالسنة )5(. اأن معدل حدوث كارسينوما املرارة يف النساء 1,5 حالة يف 100 األف نسمة بالسنة )6(. اإن الأ رقام املذكورة اأعاله تظهر معدلت احلدوث يف دول اأوروبية متعددة.‏ من املالحظ بشكل خاص اإن معدل احلدوث ملختلف اأورام الكبد تختلف يف اأماكن اأخرى 11 11
