Dementia Diagnostics F E A T U R I N G T H E B E S T O F G E R M A N M E D I C I N E وسائل تشخيص العته aspects of neurochemical analysis of dementia disorders is to find biomarkers capable to predict development of AD in patients or even appareently healthy subjects with mild memory complains. Such a pre-clinical stage is often described as Mild Cognitive Imppairment (MCI), and it is known that approximately 10% of MCI subject will develop dementia disorders within one year. In a study addressing a combbination of three CSF biomarkeers, namely tau, pTau181 and Ab42, incipient AD could be detected among patients fulfilliing the criteria for MCI with a sensitivity of almost 70% and a specificity of almost 100%, therefore suggesting a hope to discriminate the subgroup of patients with MCI who would eventually develop AD from these who would not to offer early treatment for the subjects at risk. In a recently published paper, 137 MCI patients, who underwent lumbar puncture at baseline, were followed clinically for 4-6 years with 39 healthy individuals, cognitively stable over 3 years, serving as controls, and a combination of CSF tau and Ab1-42 at baseline yielded a sensitivity of 95% and a specificity of more than 80% for detection of incipient AD in patients with MCI. In our recently published study, based on multiplexing technology, the cut off levels of Ab1-42 and pTau181 derived from the differential analysis of early dementia patients allowed correct definition of a subgroup of MCI subjects characterizing with increased risk to develop AD from a subgroup of MCI subjects without such a risk. Can we Differentiate Dementtias from One Another Alzheimer’s disease is by far not only dementia disorder, and discrimination among them is quite difficult. In a recently published study, we investiggated the Ab peptides pattern in the CSF of patients with Alzheimer’s disease, dementia with Lewy bodies (DLB), and Parkinson’s Disease dementia (PDD) to observe disease-speccific variations of the CSF Ab patterns: some of the biomarkeers discriminated all diagnostic groups from each other at a highly significant level, except DLB from PDD, and additionaally, a novel peptide with Ab-like immunoreactivity was observed constantly in the CSF of all 88 investigated patients. Similarly, we noticed some alterations suggesting possible differential diagnosis of another dementing condition: frontottemporal lobe degeneration. Dementia could also be one of the symptoms in several potenttially treatable diseases of the brain. Therefore, the American Academy of Neurology advises CSF analysis in differential diagnosis of dementia in cases suspected for: metastatic cancer, CNS infection, reactive serum syphilis serology, hydrocephalus, immunosuppresssion, CNS vasculitis, and in all patients under the age of 55. In majority of these diseases (if not all of them), and in virtually all cases suspected for inflammmation of the central nervous system, analysis of CSF is a condition sine qua non for corrrect diagnosis. Among many others, inflammmatory diseases of CNS form a group of reversible disorders often characterized with memory disturbances, and CSF analysis should be performed in all cases suspected for it. With a combination of parameters routinely analyzed in CSF and serum, neuroborreliosis can be confirmed or rejected with very reasonably accuracy. In neurosyphilis, another example of CNS inflammatory disease that can present with dementia, normal or only slight dysfunction of blood-CSF barrrier is observed, accompanied by intrathecal IgM and IgG synthesis. Serologic analyses (VDRL and TPHA) are usuaally positive and confirm the diagnosis. Infection with human immunoddeficiency virus (HIV) can affect cognitive functions in several manners. CNS involvement is a frequent complication in AIDS, appearing in 30-60% of patients. In Germany, about 11% of subjects with AIDS presented dementia.
