Urology F E A T U R I N G T H E B E S T O F G E R M A N M E D I C I N E علم املسالك البولية New Options in the Therapy of Micturition Problems Greenlight Laser Vaporisation of the Prostate and Sacral Neuromodulation خيارات جديدة يف معاجلة التبول التبخري بواسطة الليزر ذوا الضوء االأ خرض للبوستات وتعديل االأ عصاب العجزية Micturition problems is a summmary term for various urologiccal diagnoses. Especially due to its frequent occurrence the widely known prostatic hyperpplasia with its corresponding obstructive and irritative micturrition pains is one of the major micturition problems. The first part of this article will look at the greenlight laser vaporisation of the prostate as a new therapy option. The second part will focus on the subject of neuromodulattion. This refers to an electric irritation of the sacral nerves as a therapy of urgency/inconttinence and of the hypotensive bladder with residual urine formation. As the man gets older, the frequency of a prostatic hyperpplasia increases constantly so that at the age of 50 50% of all men and finally at the age of 80 almost every man shows a prostatic hyperplasia. Regardlless of the size 10 to 20% of the male population requires a therapy of the benign prostatic syndrome, as the urine stream weakens, the residual urine possibly increases and even retention of urine might occur. If medication therapy (alpha blockers, 5-alpha reductase inhhibitors) is not successful, the transurethal resection (TUR-P) is said to be the gold standard for prostate volumes of up to about 80 millilitres. Among the new therapy options the greenlight laser vaporisation has established itself in recent years as a therapy of choice. Contrary to the extraction of prostate tissue by means of TUR-P for histological preparattion, the greenlight laser vaporissation is an ablative procedure involving the vaporisation of tissue – a histological examinattion is not possible. For this procedure the new 120 watt very efficient potassium-titannyl-phosphate-laser (PTP-laser) is used, which is also called greenlight laser due to its green laser light at 532 millimettres (Fig. 1). تعترب عبارة مشاكل التبول خالصة لعدد من الأ مراض البولية.‏ خاصة حلدوثه املتكرر يعترب فرط تصنع الربوستات وما ينجم عنه من انسداد و اأمل التبول التهيجي واحد من املشاكل الكربى للتبول.‏ يتناول القسم الأ ول من هذا املوضوع عن تبخري الربوستات بواسطة الليزر ذوا الضوء الأ خرض كخيار عالجي جديد.‏ بينما يركز اجلزء الثاين على موضوع التعديل العصبي.‏ والتي تشري اإىل التخريش الكهربائي لالأ عصاب العجزية لعالج الزحري والسلس البويل,‏ واملثانة ناقصة التوتر وتشكل الثمالة البولية.‏ بينما يتقدم الرجل من العمر تزداد نسبة فرط تصنع الربوستات بشكل مستمر حيث اأنه يف عمر 50 ‏سنة هناك % 50 من الرجال مصابون بينما كل رجل يف عمر 80 ‏سنة يشكو من فرط تصنع الربوستات.بغض النظر عن احلجم فان ‎10‎‎%20‎ من جمتمع الرجال يتطلب معاجلة لتناذر الربوستات السليم،‏ بسبب ‏ضعف رشق البول,‏ فان املحتمل يزيد البول املتبقي مع احتباس بويل.‏ اذا مل تنجح املعاجلة الدوائية ‏)حارصات الفا,‏ مثبطات ‎5‎ الفا ريدكتاز(‏ فان القطع عرب الحليل .)TUR-P( يعترب املعاجلة الذهنية اذا كان حجم الربوستات حتى 80 ميلي ليرت.‏ من ‏ضمن املعاجلات احلديثة التبخري بالليزر ذو الضوء الأ خرض اعترب يف السنوات الخرية العالج المثل.‏ خالفا لستئصال نسيج الربوستات عرب التنظري TUR-P واحلصول على املحرضات النسيجية فاإنه يعترب التبخري بالليزر بالضوء الأ خرض هو اإجراء باتر يتضمن تبخري الأ نسجة.‏ ويف هذه احلالة ل ميكن اإجراء الفحوص النسيجية ولإ جراء هذه العملية فان 120 واط تعترب فعالة جدا يف استعمال الليزر فوسفات تياتيل البوتاسيوم .)PTP-laser( ويطلق عليها اأيضا اسم الليزر ذو الضوء الأ خرض حيث يكون ‏ضوئها اخرض يف درجة 532 ملم ‏)الشكل 1(. 52 52
Urology Its specialty is a high absorpttion of haemoglobin and thus an optimal arrest of bleeding. This makes it possible to carry out a prostate resection even in patients under anticoagulattion with counter indication for a TUR-P. A precondition is an adequate prostate preventtive care with PSA definition and digital rectal examination to exclude a prostate carcinnoma, since this diagnosis can be overlooked if no histology takes place. Clinical experience shows a significantly shorter stay in hospital, a reduced catheter time (on average 24 hours) and minimal blood loss. A limiting factor is a large prosttate gland of more than 60 to 70 grams (Fig. 2). But the greenlight laser vaporissation does not at all replace the TUR-P, but in case of a careful indication, it is an optimmal therapy form for patients who were forced to use a catheeter for urine outflow all their lives due to other diseases and therefore had to accept a limited quality of life. As mentioned before, the neuromodulation has an indicattion position in another area of micturition problems. Aside from urgency with incontinnence, other indications are a hypotone bladder with a weak bladder muscle and the pelvic pain syndrome. The neuromodulation is a promising option if no significant therapy success could be achieved before. In a test stimulation an electric irrritation of the nerves in the spinnal cord at the level of the sacrral bone is used to modulate the bladder and/or intestinal function.
