GENERAL REQUIREMENTS:Applicant hereby agrees to the following conditions:1. The Public Works Department will be given written notice at least five (5)working days prior to the actual start <strong>of</strong> work.2. Call ULOCO for underground utility locates and/or contact all appropriateutility companies individually prior to the beginning <strong>of</strong> construction.3. Mail delivery shall not be interrupted as a result <strong>of</strong> the installation.Temporary facilities shall be made to accommodate mail delivery. Mailboxesshall be replaced equal or superior to that condition which existed prior toconstruction.4. Install and maintain the utility in such safe and proper condition that it will notinterfere with or endanger travel upon said highway, nor obstruct norinterfere with the proper maintenance there<strong>of</strong>.5. Pavement and sidewalk replacement shall be completed as soon aspossible after backfilling and tamping operations and in ALL cases asphaltpavement cuts shall be replaced within three (3) calendar days aftercompletion has been achieved.6. All driveways shall be bored when possible or replaced to a quality equal to,or superior to, that prior to construction. The Public Works Department mustapprove any open cuts on a street or driveway.7. Drainage shall not be impeded at any time. Drainage structures damagedduring installation shall be replaced.8. Any shrubbery or landscaping affected by the installation shall be replacedto the satisfaction <strong>of</strong> the Public Works Department.9. It shall be the sole responsibility <strong>of</strong> the Applicant to maintain all pavementcuts for a period <strong>of</strong> one year.
