PROCEDURE FOR THE COMPLETION<strong>of</strong>“DRIVEWAY PERMIT”Permit Required:Permit Obtained:Permit Submitted:When Developer or Contractor proposes theinstallation <strong>of</strong> a driveway that will connect to the publicstreet system.Contact Public Works Department, Engineering Div @(336)996-5505 or copy from these specifications.Public Works DepartmentAttn: Engineering DivisionPO Drawer 728<strong>Kernersville</strong>, NC 27285Procedures:Step 1- Obtain copy <strong>of</strong> permit and complete all requiredsections.Step 2- Submit permit to the above address.Step 3 - <strong>Town</strong> Engineer will review and process requestwithin 5 days <strong>of</strong> receipt. Additional requirements may beattached to the permit after review and depending upon thesituation or location <strong>of</strong> proposed driveway.Step 4 - Approved permit will be mailed to permittee with abill for $20.00 or letter stating why permit request wasdenied.Coordination:If Permitee requires assistance or additional information,please contact the:Public Works DepartmentEngineering Division(336) 992-0304
