Theappendix is organized as follows:• Section A discusses <strong>the</strong> purpose <strong>of</strong> <strong>the</strong> survey and presents summary statisticson <strong>the</strong> survey sample.• Section B describes <strong>the</strong> sample design, including <strong>the</strong> selection <strong>of</strong> primarysampling units, clustered and unclustered components, target population, strata,and sample sizes.• Section C discusses <strong>the</strong> content and design <strong>of</strong> <strong>the</strong> survey questionnaire.• Section D describes <strong>the</strong> data collection process and procedures.• Section E presents additional information and data that support <strong>the</strong> findingspresented in Chapter III.A. OVERVIEW1. Purpose <strong>of</strong> <strong>the</strong> SurveyAs part <strong>of</strong> <strong>the</strong> TTW evaluation, MPR is conducting <strong>the</strong> National Beneficiary Survey(NBS). The survey, sponsored by SSA’s Office <strong>of</strong> Disability and Income Security <strong>Program</strong>s,collects data from a nationally representative sample <strong>of</strong> SSA disability beneficiaries(hereinafter referred <strong>to</strong> as <strong>the</strong> Representative Beneficiary Sample), and two cross-sectionalsamples <strong>of</strong> TTW participants (hereinafter referred <strong>to</strong> as <strong>the</strong> <strong>Ticket</strong> Participant Sample) acrossfour years. In addition, cohorts <strong>of</strong> TTW participants will be followed longitudinally. In all,four rounds <strong>of</strong> interviews will be conducted annually beginning in 2004. Data are collectedby means <strong>of</strong> a computer-assisted telephone interview (CATI) with a computer-assistedpersonal interview (CAPI) followup for beneficiaries who did not respond <strong>to</strong> <strong>the</strong> CATIinterview or who requested an in-person interview <strong>to</strong> facilitate <strong>the</strong>ir participation in <strong>the</strong>
