(2) Each voting member will complete a DA Form 3356–1 (USAR Board Member Appraisal Worksheet) on each<strong>Soldier</strong> being considered for promotion. Points will be awarded based on information contained in the <strong>Soldier</strong>’smilitary personnel records. Each voting member will—(a) Enter the number of points awarded in each area <strong>and</strong> the total points awarded.(b) Enter any remarks he or she deems necessary under remarks. However, if the board member is not recommendingthe <strong>Soldier</strong> for promotion, he or she will cite the reason(s) in the remarks section.(c) Mark the appropriate block to recommend or not recommend a <strong>Soldier</strong> for promotion.(3) The recorder will—(a) Collect all copies of DA Form 3356–1 <strong>and</strong> inform the president of the results of the vote if a tie exists. Theboard president will then vote to break the tie.(b) Complete DA Form 3357 (fig 3–3). Extract relative comments from the remarks section of the DA Forms3356–1 <strong>and</strong> consolidate them on DA Form 3357. If there is insufficient room, these comments may be placed on aseparate sheet of bond paper <strong>and</strong> attached to the DA Form 3357.(c) Obtain the board president’s signature on DA Form 3357 <strong>and</strong> then destroy all copies of DA Form 3356–1 so asnot to compromise an individual board member’s balloting.(d) Complete DA Form 3355, Section C, items 1 <strong>and</strong> 2; certify that the total points indicated are correct by placinghis or her typed or printed name <strong>and</strong> signature in Section D, item 3; <strong>and</strong> complete items 1, 2, 4, <strong>and</strong> 5.(4) The convening (promotion) authority will certify that the <strong>Soldier</strong> has been recommended by a valid promotionboard by placing his or her signature on DA Form 3355, Section D, item 7, <strong>and</strong> completing items 6 <strong>and</strong> 8.d. Adjournment. On completing the actions described in 5–14a through d:(1) The convening authority may advise the president to adjourn the board.(2) The recorder will insert the completed DA Form 3355 in the appropriate <strong>Soldier</strong>’s promotion packet.5–15. Report of promotion selection board proceedingsAfter the board has adjourned, the following actions will be taken:a. The recorder will prepare a report of board proceedings for the president of the board using the format at figure5–1. This report is exempt from report control ruling under AR 335–15, paragraph 7–2h. The report will include—(1) General information to include where <strong>and</strong> when the board met.(2) Membership of the board.(3) Alphabetical listing, by name, within recommended grades (SGT <strong>and</strong> SSG) of all <strong>Soldier</strong>s the board consideredfor promotion.(4) Names <strong>and</strong> SSNs of <strong>Soldier</strong>s recommended for promotion by a majority of the voting members. Names will belisted according to recommended grade <strong>and</strong> zone <strong>and</strong> descending promotion scores in the format shown in figure 5–1,page 3. This list will identify only those <strong>Soldier</strong>s who attained scores equal to or greater than—(a) 450, if competing for promotion to SSG.(b) 350, if competing for promotion to SGT.b. The president of the board will review the report of board proceedings (fig 5–1, page 1) for accuracy <strong>and</strong> affix hisor her signature.c. The report of board proceedings with the recommended list, promotion board appointment, <strong>and</strong> promotion packetsenclosed will be sent to the convening (promotion) authority.5–16. Actions by the convening (promotion) authority following receipt of the board proceedingsa. Upon receipt of the board proceedings the convening (promotion) authority will approve or disapprove thepromotion board proceedings within 15 days after the date of the adjournment of the promotion selection board.Approval or disapproval only pertains to the correct constitution <strong>and</strong> conduct of the board. Disapproval will not beused to disagree with the board’s recommendations. The convening authority will take one of the following actions:(1) Approve the report in its entirety <strong>and</strong> authorize the names of recommended <strong>Soldier</strong>s to be integrated into thepermanent recommended promotion list. A <strong>Soldier</strong>’s promotion selection date is the date the convening authorityapproves the board proceedings, regardless of the date the board met.(2) Disapprove the report in its entirety <strong>and</strong> convene a new promotion selection board, composed of new membersper paragraph 5–13, following the procedures in paragraph 5–14. The new board will consider all <strong>Soldier</strong>s who wereconsidered by the disapproved promotion selection board (both recommended <strong>and</strong> not recommended <strong>Soldier</strong>s) <strong>and</strong> filea new report as required by this paragraph. In addition, the convening (promotion) authority will advise all <strong>Soldier</strong>swho were considered (whether or not recommended) by the disapproved board of the reason(s) why the report of boardproceedings was not approved. For those <strong>Soldier</strong>s who have departed the comm<strong>and</strong> before the second board isconvened, they will be notified by memor<strong>and</strong>um that the report of board proceedings was not approved.b. The convening authority will—(1) File the original report of promotion board proceedings with DA Form 3357 for 2 years.72 AR 600–8–19 • 20 March 2008
