Objetivos: Analizar <strong>el</strong> impacto <strong>de</strong> <strong>la</strong> comorbilidad en <strong>el</strong> beneficio<br />
<strong>de</strong> pacientes con Hepatitis C(VHC) durante <strong>el</strong> primer año <strong>de</strong><br />
tratamiento con antivirales <strong>de</strong> acción directa(DAA).<br />
Métodos: <strong>Estudio</strong> multicéntrico nacional, incluyendo 599 pacientes<br />
con VHC (cirrosis 78%; genotipo 1 82%) al iniciar tratamiento<br />
con DAA. Evaluación <strong>de</strong> comorbilidad: a) Índice Charlson abreviado<br />
(ICa); b) Índice CirCom (específico <strong>para</strong> cirrosis). Eventos adversos<br />
(EA) recogidos <strong>de</strong>s<strong>de</strong> <strong>el</strong> inicio <strong>de</strong> tratamiento: a) hospitalización (><br />
24 h); b) observación (< 24 h); c) urgencias.<br />
Resultados: El 15% (90/599) <strong>de</strong> los pacientes presentaron EA (8<br />
exitus), requiriendo ingreso 37,8% (34/90), observación 13,3% (12/90)<br />
y urgencias 48,9% (44/90). ICa (0,9 ± 1,1 vs 0,4 ± 0,8; p = 0,0001) y<br />
CirCom (0,7 ± 0,9 vs 0,3 ± 0,7; p = 0,0001) fueron más altos en pacientes<br />
con EA. Los pacientes con ICa = 1 (26,4% (47/178) vs 10,2%<br />
(43/420) (logRank 23,18; p = 0,0001); 26,8% (41/153) vs 11,5%<br />
(36/314) en cirróticos) y Circom = 1 (29,8% (36/121) vs 11,8% (41/346)<br />
(logRank 20,01; p = 0,0001)) tuvieron un riesgo mayor. Edad (60 ± 11<br />
vs 57 ± 10 años; p = 0,014), albúmina (3,8 ± 0,5 vs 4 ± 0,5 mg/dl; p =<br />
0,004), MELD (9,1 ± 2,6 vs 8,3 ± 2,4; p = 0,024) y <strong>la</strong> <strong>de</strong>scompensación<br />
previa al tratamiento (29,5% (33/112) vs 11,9% (58/487) (logRank<br />
17,86; p = 0,0001) se asociaron también con <strong>la</strong> presencia <strong>de</strong> EA. Ajustando<br />
por edad y MELD, <strong>el</strong> ICa [HR 2,69 (IC95% 1,64-4,40); p = 0,0001]<br />
albúmina [HR 0,62 (IC95% 0,40-0,96); p = 0,032] y cirrosis <strong>de</strong>scompensada<br />
[HR 1,76 (IC95% 1,04-2,99); p = 0,035] se asociaron <strong>de</strong> manera<br />
in<strong>de</strong>pendiente a <strong>la</strong> aparición <strong>de</strong> EA. Mediante data mining, <strong>de</strong>sarrol<strong>la</strong>mos<br />
un algoritmo <strong>de</strong> predicción <strong>para</strong> i<strong>de</strong>ntificar diferentes grupos<br />
<strong>de</strong> riesgo (v. figura en pág. siguiente).<br />
Conclusiones: Los pacientes con VHC con mayor comorbilidad<br />
basal tienen más riesgo <strong>de</strong> sufrir eventos adversos durante los primeros<br />
meses <strong>de</strong> tratamiento antiviral. El índice <strong>de</strong> Charlson permite<br />
i<strong>de</strong>ntificar, <strong>de</strong> manera rápida y objetiva, <strong>el</strong> beneficio global <strong>de</strong>l<br />
tratamiento <strong>para</strong> <strong>el</strong> VHC, más allá <strong>de</strong> <strong>la</strong> erradicación <strong>de</strong>l virus.<br />
