Introducción: Los pacientes con cirrosis por VHC genotipo 3 presentan<br />
tasas <strong>de</strong> respuesta virológica sostenida (RVS) subóptimas al<br />
tratamiento con agentes antivirales directos (AAD). El tratamiento<br />
durante 24 semanas con <strong>la</strong> combinación <strong>de</strong> sofosbuvir (SOF) y dac<strong>la</strong>tasvir<br />
(DCV) o ledipasvir (LDV) asociado a ribavirina (RBV) pue<strong>de</strong><br />
aumentar <strong>la</strong> tasa <strong>de</strong> RVS No existen datos <strong>de</strong> eficacia <strong>de</strong> <strong>la</strong> combinación<br />
con ledipasvir durante 24 semanas ni com<strong>para</strong>tivos <strong>de</strong> ambas<br />
pautas en esta pob<strong>la</strong>ción.<br />
Objetivos: Com<strong>para</strong>r <strong>la</strong> RVS12 entre SOF+DCV+/-RBV 12 o 24 semanas<br />
y SOF+LDV+RBV durante 24 semanas.<br />
Métodos: <strong>Estudio</strong> multicéntrico observacional <strong>de</strong> una cohorte <strong>de</strong><br />
pacientes cirróticos genotipo 3 tratados en práctica real en 9 hospitales<br />
<strong>de</strong> <strong>la</strong> Comunidad <strong>de</strong> Madrid, tratados con SOF en combinación<br />
con DCV o LDV más RBV. Se incluyeron pacientes con diagnóstico<br />
clínico o histológico <strong>de</strong> cirrosis o e<strong>la</strong>stografía <strong>de</strong> transición ><br />
12,5 Kpa, > 18 años, naïve o previamente tratados y se excluyeron<br />
pacientes coinfectados con VHB o VIH.<br />
Resultados: Se han incluido 162 pacientes (130H; 32M), edad<br />
media 55,4 años (8,5 DE), previamente tratados con peginterferón<br />
más RBV un 46%. La mediana <strong>de</strong> MELD score fue 8 puntos (DE 3,8) y<br />
<strong>la</strong> carga viral basal fue 1,4 × 10 6 (DE 8,1 × 10 6 ).La proporción <strong>de</strong><br />
pacientes con esca<strong>la</strong> <strong>de</strong> MELD > 10 puntos, p<strong>la</strong>quetas < 75.000 y<br />
albúmina sérica < 3,5 fue <strong>de</strong> 22%, 21% y 20%, respectivamente. La<br />
distribución <strong>de</strong> los pacientes por CPT A/B/C fue 130/28/4. La rigi<strong>de</strong>z<br />
hepática medida mediante e<strong>la</strong>stografía <strong>de</strong> transición fue > 21,1<br />
Kpa en un 43% <strong>de</strong> los pacientes. Noventa y nueve pacientes se trataron<br />
con DCV y sesenta y tres con LDV. El 91% recibió RBV y un 88%<br />
tratamiento durante 24 semanas. Los pacientes tratados con DCV<br />
presentaron MELD score > 10 y albumina < 3,5 g/dl en mayor proporción<br />
que los tratados con LDV. Se dispone <strong>de</strong> datos <strong>de</strong> RNA-VHC<br />
en semana 4 <strong>de</strong> tratamiento en 93/130 pacientes y <strong>el</strong> 71% tuvieron<br />
RNA VHC in<strong>de</strong>tectable (RV4) sin diferencias entre <strong>el</strong> tipo <strong>de</strong> inhibidor<br />
N5A (71% DCV vs 73% LDV) tras ajustar por función hepática.<br />
Hasta <strong>la</strong> fecha, sólo dos pacientes suspendieron precozmente <strong>el</strong><br />
tratamiento por falta <strong>de</strong> adherencia y un paciente grado C CPT falleció<br />
durante <strong>el</strong> tratamiento. De los 78 pacientes que completaron<br />
<strong>el</strong> tratamiento, todos excepto 1 (98,7%) alcanzaron respuesta al<br />
final <strong>de</strong> tratamiento (RFT). De los 18 pacientes que completaron <strong>el</strong><br />
seguimiento, 15/17 con DCV, y 1/1 con LDV obtuvieron RVS12<br />
(89%).<br />
Conclusiones: El tratamiento con SOF y DCV o LDV durante 24<br />
semanas en pacientes con cirrosis y genotipo 3 alcanza tasas cercanas<br />
al 90%. A pesar <strong>de</strong> que DCV se ha asociado con mayor potencia<br />
antiviral in vitro frente al genotipo 3 com<strong>para</strong>do con LDV, no hay<br />
diferencias significativas entre ambas combinaciones intratratamiento<br />
(RV4). Los resultados <strong>de</strong> RVS12 se aportarán tras completar<br />
<strong>el</strong> período <strong>de</strong> seguimiento.
