Cette entité a déjà été rapportée chez les sportifs au niveau du rachiscervical. Aucun cas de médullopraxie dorso-lombaire n’aurait été à notreconnaissance décrit.ButRapporter deux observations de neurapraxie du rachis dorso-lombaire etessayer de les distinguer des cas auparavant décrits de médullopraxie cervicale.ObservationsDeux jeunes sportifs âgés de 21 et 19 ans, ont présenté suite à un accidentsportif une paraplégie sensitivo-motrice complète sans troubles sphinctériens,spontanément réversible après quelques heures avec une récupérationcomplète au bout de quelques jours. L’imagerie standard était normale dansles 2 cas. Le diagnostic était confirmé chez le deuxième patient par une IRMmontrant une anomalie de signal centro-médullaire intéressant la partie postérieurede la moelle dorsale et par les critères cliniques, surtout évolutifs, del’accident chez le premier malade.ConclusionLa neurapraxie dorso-lombaire semble présenter des similitudes mais surtoutdes différences avec celle de siège cervical. Elle paraît se distinguerpar l´absence de facteur favorisant, notamment d´un canal médullaire étroitconstitutionnel ou d’une instabilité du canal vertébral, et par le délai de récupérationplus long. D’autres études à plus large échelle sont nécessairesafin d’élucider les aspects pathomécaniques et physiopathologiques de cetteentité encore peu connue.ABsTrAcTBackgroundSpinal cord neurapraxia is a transitory and totally reversible disruption ofnerve conduction due to acute spinal cord compression. It represents an entitythat has already been reported in cervical localisation in athletes. To thebest of our knowledge, it has never been previously described in dorso-lumbarspinal cord.Aim of the presentationWe report here in two original observations of dorso-lumbar spinal cord neurapraxiaand try to distinguish it from the cervical one.ObservationsTwo 21 and 19-year-old young athletes presented following sports accidentwith a complete motor and sensory paraplegia without any sphincteriantrouble, spontaneously reversible with a full recovery after few days. Conventionalradiographies were normal in the 2 cases. Diagnosis was confirmed inthe second patient by nuclear magnetic resonance imaging showing a centromedullarsignal anomaly in the posterior dorsal spinal cord and by clinicalpresentation especially the favourably spontaneous outcome in the first case.ConclusionDorso-lumbar spinal cord neurapraxia have some similarities with cervicalneurapraxia but also seems to distinguish from it. In the dorso-lumbar form,there is no promoting factor, such as a constitutional narrowed medullar canalor vertebral instability, and recovery seems to be longer.
