Elle pose un problème d’indication et <strong>de</strong> choix thérapeutique<strong>qui</strong> dépend <strong>de</strong> plusieurs facteurs. L’ostéotomie tibiale <strong>de</strong> valgisationconstitue un outil thérapeutique d’un apport non négligeable, notamment<strong>pour</strong> les sujets jeunes actifs avec arthrose débutante. Par ailleurs ces indicationspeuvent s’é<strong>la</strong>rgir au dépend <strong>de</strong> l’âge et du sta<strong>de</strong> évolutif.Le but <strong>de</strong> notre travail est d’évaluer les résultats <strong>de</strong> l’ostéotomie tibiale <strong>de</strong>valgisation chez les sujets âgés <strong>de</strong> plus <strong>de</strong> 60 ans à court et à long termeet <strong>de</strong> préciser les facteurs pronostiques <strong>qui</strong> peuvent modifier les résultats.Matériel et Métho<strong>de</strong>sNous rapportons une étu<strong>de</strong> rétrospective <strong>de</strong> 62 genoux <strong>pour</strong> 52 patients âgés<strong>de</strong> plus <strong>de</strong> 60 ans et colligés dans le service d’Orthopédie Traumatologie <strong>de</strong>l’hôpital Aziza Othmana entre 1985 et 2002. Nous avons retenu <strong>pour</strong> cetteétu<strong>de</strong> les patients ayants un recul minimum <strong>de</strong> 5 ans et un bi<strong>la</strong>n radio-cliniquecomplet. Tous nos patients consultaient <strong>pour</strong> <strong>de</strong>s gonalgies bi<strong>la</strong>térales,<strong>la</strong> moitié d’entre eux étaient tarés. Sur le p<strong>la</strong>n radiologique, le tiers <strong>de</strong>s patientsavaient un sta<strong>de</strong> d’arthrose évoluée, le compartiment fémoro-patel<strong>la</strong>ireet externe ont été atteint dans respectivement 79% et 60% et le varumglobal moyen était <strong>de</strong> 11,3°.RésultatsA un recul moyen <strong>de</strong> 7 ans et ½ et selon <strong>la</strong> cotation fonctionnelle <strong>de</strong> Guepar,nos résultats ont été jugés bons dans 70% <strong>de</strong>s cas avec dé<strong>la</strong>i <strong>de</strong> satisfactionmoyen <strong>de</strong> 5 ans et ½.L’analyse statistique <strong>de</strong> nos résultats avec le logiciel « SPSS» a permis <strong>de</strong>mettre en exergue certains facteurs déterminants à savoir les tares associées,le <strong>de</strong>gré <strong>de</strong> déformation en varus, le sta<strong>de</strong> d’arthrose et <strong>la</strong> correctionangu<strong>la</strong>ire.ABsTrAcTIntroductionKnee arthritis is a frequent pathology in our country. The indications and thetherapeutic choice <strong>de</strong>pend on several factors. The high tibial valgus osteotomyhas a consi<strong>de</strong>rable contribution, in particu<strong>la</strong>r for the active young subjectswith osteoarthritis beginner. In addition it can be exten<strong>de</strong>d on <strong>de</strong>pendof age and the evolution stage.The aim of our paper was to evaluate the outcome of the high tibial valgusosteotomy in patients upper than 60 years of age to specify the prognosisfactors which can modify the results.Material and MethodsWe report a retrospective study of 62 knees for 52 patients ol<strong>de</strong>r than 60years and colliged in the <strong>de</strong>partment of Orthopaedic and Traumatologic surgeryof Aziza Othmana hospital between 1985 and 2002. We retained for thisstudy the patients having a follow up of 5 years and complete roentgenograms.All our patients consulted for bi<strong>la</strong>teral knee pain, half of them have anassociated pathology. On the radiological level, one the third of the patientshad an important stage of osteoarthritis, the fémoro-patel<strong>la</strong>r and <strong>la</strong>teral kneecompartment were damaged in respectively 79% and 60% and the varusmeans were of 11.3°.ResultsWith a follow up of 7 years and a half and according to the functional evaluationof GUEPAR group, the results were consi<strong>de</strong>red to be good in 70% of thecases with average time of satisfaction of 5 years and a half.The statistical analysis put forward certain <strong>de</strong>termining factors: associatedpathology, <strong>de</strong>gree of varus <strong>de</strong>formation, stage of osteoarthritis and angu<strong>la</strong>rcorrection.mots-clés : arthrose, genou, sujet âgé, ostéotomie, tibia.Keywords: osteoarthritis, knee, osteotomy, tibia143

Il s’agit d’une ma<strong>la</strong>die évolutive ;non traitée le pronostic fonctionnel est sombreavec <strong>de</strong>struction articu<strong>la</strong>ire et par conséquenceun handicap majeur. L’ostéotomie tibiale <strong>de</strong> valgisations’impose comme traitement <strong>de</strong> choixchez le sujet jeune avec arthrose débutante [11,13, 26].La prise en charge <strong>de</strong> cette pathologie est souventretardée dans notre pays du fait d’une réticence<strong>de</strong>s patients, <strong>de</strong> leurs conditions socio-économiqueset <strong>de</strong> l’insuffisance <strong>de</strong> <strong>la</strong> couverture sociale.On se trouve généralement <strong>de</strong>vant <strong>de</strong>s patientssouvent âgés, tarés avec un sta<strong>de</strong> d’arthroseévolué et une déformation importante du genou.Mais que faire lorsque le patient dépasse l’âge légal<strong>de</strong> <strong>la</strong> retraite (60 ans) et/ou qu’ils présententune déformation angu<strong>la</strong>ire importante et un sta<strong>de</strong>d’arthrose évoluée ? Le remp<strong>la</strong>cement prothétiqueest il <strong>la</strong> seule indication possible ? Et si le ma<strong>la</strong><strong>de</strong>est <strong>de</strong> condition socio-économique défavorable ?Ce travail se propose <strong>de</strong> faire une étu<strong>de</strong> rétrospectiveà propos <strong>de</strong> 62 genoux opérés chez 52patients âgés <strong>de</strong> plus <strong>de</strong> 60 ans dont l’objectifessentiel est <strong>de</strong> faire une étu<strong>de</strong> épidémiologique,clinique, et radiologique afin d’évaluer les résultatsimmédiats et à distance avec un recul minimum<strong>de</strong> 5 ans, <strong>de</strong>l’otéotomie tibiale <strong>de</strong> valgisation.ii. matériel et métho<strong>de</strong>sA- MatérielSur un total 108 dossiers colligés <strong>pour</strong> cetteétu<strong>de</strong> rétrospective entre 1985 et 2002 à l’hôpitalAziza Othmana, 52 ont été retenus soit 62ostéotomies tibiales <strong>de</strong> valgisation et répondantsaux critères d’inclusions (un dossier clinique etradiologique complet et un recul minimum <strong>de</strong>5 ans).1. Données épidémiologiquesL’âge moyen était <strong>de</strong> 66 ½ ans avec <strong>de</strong>s extrêmesentre 60 et 78 ans. La répartition <strong>de</strong>spatients selon les tranches d’âge trouvait que46% étaient âgés <strong>de</strong> plus <strong>de</strong> 65 ans. Une netteprédominance féminine a été notée avec 40femmes <strong>pour</strong> 12 hommes. Le côté gaucheétait légèrement plus touché et seulement 10patients étaient opérés <strong>de</strong>s <strong>de</strong>ux côtés. Lesconditions socio-économiques <strong>de</strong>s patientsjugées à travers une fiche sociale préétabliedans le service étaient mauvaises dans 42%<strong>de</strong>s cas. La particu<strong>la</strong>rité <strong>de</strong> cette série <strong>de</strong> patientsre<strong>la</strong>tivement âgés est qu’elle s’adressaità une popu<strong>la</strong>tion re<strong>la</strong>tivement vulnérable ettarée, 46% <strong>de</strong>s ma<strong>la</strong><strong>de</strong>s avaient au moins unetare (Tableau 1). Données cliniquesLa durée d’évolution <strong>de</strong> <strong>la</strong> symptomatologieclinique <strong>de</strong> <strong>la</strong> gonarthrose avant sa priseen charge était en moyenne <strong>de</strong> 8,5 ans avec<strong>de</strong>s extrêmes <strong>de</strong> 6 mois à 20 ans. Les gonalgiesconstituaient le motif <strong>de</strong> consultation et<strong>la</strong> p<strong>la</strong>inte fonctionnelle <strong>la</strong> plus importante.L’évaluation <strong>de</strong> <strong>la</strong> douleur s’est faite sur <strong>de</strong>scritères subjectifs en 4 sta<strong>de</strong>s (Tableau 2). Ellesétaient le plus souvent <strong>de</strong> type mixte et qualifiées<strong>de</strong> majeures <strong>pour</strong> 92% <strong>de</strong>s patients.tableau 2 : Évaluation <strong>de</strong> <strong>la</strong> douleur du genou en 4 sta<strong>de</strong>s.sta<strong>de</strong> 1sta<strong>de</strong> 2sta<strong>de</strong> 3sta<strong>de</strong> 4Douleur nulleDouleur modérée : occasionnelle à l’effort, barométrique,n’interférant pas sur l’activité physique et ne nécessitant aucuneprise médicamenteuseDouleur importante : discontinue réduisant l’activité physiqueet/ou imposant <strong>la</strong> prise <strong>de</strong> médicamentsDouleur permanente : à <strong>la</strong> marche et/ou nocturne, sévère, limitantconsidérablement l’activité physiqueLe périmètre <strong>de</strong> marche était limité entre 100et 500 mètres dans 72% <strong>de</strong>s cas. Bien quece paramètre témoigne <strong>de</strong> l’importance et <strong>de</strong>l’évolutivité <strong>de</strong> l’arthrose du genou, il restecependant multifactorielle surtout <strong>pour</strong> cettetranche <strong>de</strong> popu<strong>la</strong>tion âgée et où les tares(cardio-vascu<strong>la</strong>ires, respiratoires, neurologiques…)y jouent un rôle non négligeable.Dans cette série <strong>la</strong> flexion du genou était rarementaltérée, cependant un flexum du genouatteint était noté dans 28 genoux. Celui-ci étaitsupérieur à 15° <strong>pour</strong> 5 genoux. D’autre part15 genoux souffraient d’une <strong>la</strong>xité ligamentaireassociée, elle était périphérique <strong>la</strong>téraleexterne <strong>pour</strong> 80% d’entre eux et antérieure<strong>pour</strong> le reste. La quasi-totalité <strong>de</strong>s patientsprésentaient un syndrome fémoro-patél<strong>la</strong>ireassociée à l’atteinte fémoro-tibiale.3. Données radiologiques :Le bi<strong>la</strong>n radiologique comportait <strong>pour</strong> tous lespatients une radiographie standard <strong>de</strong>s <strong>de</strong>uxgenoux face et profil en charge en pré, postopératoire,à <strong>la</strong> consolidation et eu recul, <strong>de</strong>s
