La maturation <strong>de</strong> l’ostéome a été jugée sur l’aspectradiologique d’ostéome mature, sur <strong>la</strong> stabilisationdu quotient <strong>de</strong> captation à <strong>la</strong> scintigraphieau technétium 99 lors <strong>de</strong> <strong>de</strong>ux examensespacés <strong>de</strong> 6 mois et sur <strong>la</strong> durée d’évolutionsupérieure à 18 mois.Aucun patient n’a eu <strong>de</strong> dosage <strong>de</strong>s phosphatasesalcalines, ni <strong>de</strong> l’hydroxyprolinurie ni <strong>de</strong> <strong>la</strong>créatine phosphokinase.Résultats du traitement chirurgical <strong>de</strong>s ostéomes neurogènes <strong>de</strong> <strong>la</strong> hanche. à propos <strong>de</strong> 30 casDans 6 cas, <strong>la</strong> résection <strong>de</strong> l’ostéome a été suivied’une arthrop<strong>la</strong>stie totale <strong>de</strong> <strong>la</strong> hanche.La voie d’abord était guidée par le siège <strong>de</strong> l’ostéome(Tableau 3). La voie <strong>de</strong> Enneking a été utilisée<strong>pour</strong> les ostéomes circonférentiels 5 fois sur8. Tous les patientsont été adressés à leur sortie du serviceau service <strong>de</strong> rééducation. Les anticoagu<strong>la</strong>nts(HBPM) étaient systématiquement prescrits pendant3 semaines. L’indométacine (100 mg/j pendant21 jours) dans le but d’éviter les récidivesn’a été utilisée que chez 8 patients (27%). Aucunpatient n’a eu <strong>de</strong> radiothérapie post-opératoire.Figure 1 a,b : Ostéome neurogène postérieur <strong>de</strong> <strong>la</strong> hanche droite. Difficultéslors <strong>de</strong> l’instal<strong>la</strong>tion et ankylose articu<strong>la</strong>ire en position vicieuseB- Métho<strong>de</strong>sL’analyse <strong>de</strong>s résultats a porté sur les complicationsprécoces, sur l’apparition <strong>de</strong> récidive <strong>de</strong> l’ostéomeet sur le résultat anatomique et fonctionnel évaluéà 34 mois <strong>de</strong> recul moyen (1-10 ans).Le résultat fonctionnel a été jugé sur :• Le gain absolu en mobilité, différence entremobilité pré-opératoire et mobilité au <strong>de</strong>rnierrecul.• La perte <strong>de</strong> mobilité entre l’état per-opératoireet <strong>la</strong> mobilité au <strong>de</strong>rnier recul.• Le score <strong>de</strong> Postel et Merle d’Aubigné (PMA)évaluant <strong>la</strong> douleur, <strong>la</strong> mobilité et <strong>la</strong> stabilité.Figure 1 c,d : Délimitation première <strong>de</strong> l’ostéome puis résection auciseau à frapperiii. résultatsA- ComplicationsDans quatre cas, une fracture per-opératoire estsurvenue : sous trochanterienne (1 cas), col fémoral(1 cas), tête fémorale (1 cas), tête et paroiantérieure du cotyle (1 cas). Ces fractures ont ététraitées par ostéosynthèse sauf <strong>la</strong> fracture du col<strong>qui</strong> a été traitée par arthrop<strong>la</strong>stie totale.Un patient a eu une brèche <strong>de</strong> l’artère fémoraleper-opératoire et est décédé dans un tableau <strong>de</strong>coagu<strong>la</strong>tion intravascu<strong>la</strong>ire disséminée. Il s’agissaitd’un traumatisé médul<strong>la</strong>ire <strong>qui</strong> avait 4 localisations(2 hanches et 2 genoux).173

Les pertes sanguinesont été estimées à 2,7 g/dl en moyenne(0,9 - 10,3).Deux sepsis profonds ont eu une évolution favorableaprès drainage chirurgical et antibiothérapieprolongée.Quatre hanches étaient instables en post-opératoire.Il s’agissait <strong>de</strong> résection d’ostéomes circonférentielspar voie <strong>de</strong> Enneking, dont 3 avecarthrop<strong>la</strong>stie totale. La réduction et <strong>la</strong> tractiontrans osseuse ont permis d’obtenir une hanchestable dans les quatre cas.Une récidive <strong>de</strong> l’ostéome a été notée dans 5 cas(trois patients), toujours précoce (entre 1 et 3 mois).Deux patients ont eu une récidive bi<strong>la</strong>térale : lepremier, <strong>qui</strong> avait 5 localisations d’ostéomes gardait<strong>de</strong>s séquelles neurologiques sévères (sta<strong>de</strong>III <strong>de</strong> Gar<strong>la</strong>nd) et s’était compliqué d’un sepsis.Le <strong>de</strong>uxième gardait une tétra-parésie. Bon résultat à droite, récidive <strong>de</strong> l’ostéomeà gauche.Les 6 patients confinés à <strong>la</strong> chaise rou<strong>la</strong>nte ontpu déambuler, à l’ai<strong>de</strong> <strong>de</strong> cannes <strong>pour</strong> trois patients(Fig 4) et sans cannes <strong>pour</strong> les trois autres.174La perte <strong>de</strong> mobilité était <strong>de</strong> 21,5° <strong>pour</strong> <strong>la</strong>flexion, 5° <strong>pour</strong> l’abduction et 4° <strong>pour</strong> <strong>la</strong> rotationexterne.Le score PMA était satisfaisant (>14) dans 46%<strong>de</strong>s cas. Les bons résultats n’étaient pas corrélésau nombre d’ostéomes mais au sta<strong>de</strong> <strong>de</strong> Gar<strong>la</strong>nd.Ainsi 8 <strong>de</strong>s 9 cas c<strong>la</strong>ssés sta<strong>de</strong> II <strong>de</strong> Gar<strong>la</strong>ndont obtenu un résultat (PMA) satisfaisantalors que 7 <strong>de</strong>s 8 résultats médiocres ou mauvaisétaient <strong>de</strong>s sta<strong>de</strong>s III <strong>de</strong> Gar<strong>la</strong>nd.figure 4a : Ostéome hanche droite après un coma chez un traumatisécrânien
