Introduction<br />

Thank you very much for participating in this survey <strong>of</strong> <strong>HUD</strong>-approved housing counseling agencies.<br />

This survey is being conducted for the U.S. Department <strong>of</strong> <strong>Housing</strong> <strong>and</strong> Urban Development (<strong>HUD</strong>)<br />

by Matrix Performance Consulting Inc. as part <strong>of</strong> a larger research project being conducted by Abt<br />

Associates.<br />

• The estimated time to complete this survey is 45 to 60 minutes, depending upon the scale <strong>of</strong><br />

counseling services provided by your organization <strong>and</strong> upon the amount <strong>of</strong> preparation done<br />

using the “Heads up” list previously forwarded to you via email.<br />

• Participation in this Government-sponsored survey is voluntary.<br />

• Please be aware that the information provided in this survey will be kept strictly confidential.<br />

The names or other identifying information for either individuals or organizations that respond<br />

to this survey will not be used in any published reports or datasets nor will this identifying<br />

information be shared with <strong>HUD</strong>.<br />

• IMPORTANT: The last section on Organizational Challenges <strong>and</strong> Policy Questions should<br />

be completed by the Executive Director or the primary person responsible for managing<br />

housing counseling for your agency.<br />

The questions included in the survey have been reviewed by the Office <strong>of</strong> Management <strong>and</strong> Budget<br />

(OMB) under the Paperwork Reduction Act <strong>of</strong> 1965 (OMB Control # 2528-0249, expiration date<br />

7/31/2010).<br />

IMPORTANT NOTES:<br />

• SAVE/RESUME CAPABILITY: You may close the survey <strong>and</strong> complete it at a later time or<br />

close it temporarily in order to forward it to another staff member for them to complete part <strong>of</strong><br />

the survey. In order for your answers on a particular page to be saved, you must click the<br />

“Next Page” button at the bottom <strong>of</strong> the page. To re-enter the survey, click the link on the e-<br />

mail invitation <strong>and</strong> use the assigned ID.<br />

• FREE RESPONSE ITEM: We are aware that the survey will not ask specific questions about<br />

all <strong>of</strong> the housing counseling <strong>and</strong> education matters that are important to you. We have thus<br />

included a free response item for your comments on housing counseling challenges <strong>and</strong><br />

policy issues at the end <strong>of</strong> the survey. As you complete the survey, you may wish to make<br />

notes about <strong>HUD</strong>’s role in supporting the housing counseling industry, challenges your<br />

agency faces, or any other significant concerns not captured by this survey <strong>and</strong> pass them<br />

along to the person ultimately responsible for submitting this survey.<br />

• REVIEW/SUBMIT SCREENS: At the completion <strong>of</strong> the final question, you will be allowed to<br />

review your responses. This will also allow you to return to some or all <strong>of</strong> the unfinished<br />

items to be sure that no items were left incomplete unintentionally. Once you have reviewed<br />

these questions again, you can choose to submit the survey by selecting “SUBMIT SURVEY”<br />

or select the option “SURVEY NOT COMPLETE” which indicates that another person in your<br />
