Homeless (men) 6-month transitional program. (1 st 30 days case management,<br />

psychosocial assessment, and individual service plan. Workshops on anger<br />

management, domestic violence, sexual, physical, mental, emotional abuse.<br />

Eligibility: (18 yrs +) No serious medical problems, negative drug test, police<br />

clearance, negative TB test, psychosocial report if applicant has a mental illness.<br />

Accepts agency referrals and walk-ins. All referrals pending final approval by Blair<br />

House intake staff. Incarcerated applicants submit Blair House admission form to<br />

Intake Coordinator for provisional acceptance. A 2 nd personal interview will be<br />

arranged upon applicant’s release. Call to schedule interview.<br />
