34 2. Statement of Assurance from the Board Royal Devon and Exeter NHS Foundation Trust Quality Report 2012/13 Review of services During 2012/13 the Royal Devon & Exeter NHS Foundation Trust provided and/or subcontracted 42 relevant health services. The Royal Devon & Exeter NHS Foundation Trust has reviewed all the data available to them on the quality of care in 42 of these relevant health services. The income generated by the relevant health services reviewed in 2012/13 represents 100% of the total income generated from the provision of relevant health services by the Royal Devon & Exeter NHS Foundation Trust. Audit – participation in clinical audits During 2012/13 38 national clinical audits and 5 national confidential enquiries covered relevant health services that the Royal Devon & Exeter NHS Foundation Trust provides. During 2012/13 the Royal Devon & Exeter NHS Foundation Trust participated in 92% national clinical audits and 100% national confidential enquiries of the national clinical audits and national confidential enquiries which it was eligible to participate in. The national clinical audits and national confidential enquiries that the Royal Devon & Exeter NHS Foundation Trust was eligible to participate in during 2012/13 are listed in Annex C. The national clinical audits and national confidential enquiries that the Royal Devon & Exeter NHS Foundation Trust participated in, and for which data collection was completed during 2012/13, are listed alongside the number of cases submitted to each audit or enquiry as a percentage of the number of registered cases required by the teams of that audit or enquiry, listed in Annex C. The reports of 10 national clinical audits were reviewed by the provider in 2012/13 and the Royal Devon and Exeter NHS Foundation Trust intends to take the following actions to improve the quality of healthcare provided as detailed in Annex D. The reports of 31 local clinical audits were reviewed by the provider in 2012/13 and the Royal Devon & Exeter NHS Foundation Trust intends to take the actions to improve the quality of healthcare as detailed in Annex E.
2. Statement of Assurance from the Board Royal Devon and Exeter NHS Foundation Trust Quality Report 2012/13 35 Clinical Audit Prize 2013 Clinical audit is a quality improvement process that seeks to make improvements in patient care. The Clinical Audit Prize 2013 was held to recognise and celebrate the achievements made by clinical staff through clinical audit and to promote high quality clinical audit projects. Fifteen entries from across the Trust were received; four were shortlisted based on both the design of the audit and the demonstrable improvements made in patient care. The shortlisted entries were: • The Management of Diabetic Ketoacidosis (DKA) – this demonstrated significant improvements in the clinical care of patients with DKA including reducing inpatient stay, increasing physician review of these patients, and improving medication management • Day Case Tonsillectomies – this showed a significant increase in the number of tonsillectomy procedures performed as day cases, which in turn increased patient comfort and experience and improved patient flow through the day case surgery wards. This also had a considerable cost benefit to the Trust in terms of efficiency savings and meeting our Department of Health ‘Payment By Results’ targets • Emergency Laparotomy – this audit improved the care pathway for these patients by fast tracking radiology reporting, getting patients to theatre faster, improving the timeliness of antibiotic administration and senior medical review for deteriorating patients, and introducing new medical equipment to measure cardiac output. All of these actions will contribute to improving the mortality rates for these patients in the long-term • Pregnancy Testing before Surgery – the audit introduced a new safe surgery checklist which brought about a significant improvement in the numbers of women having the possibility of pregnancy excluded by means of a pregnancy test prior to surgery. This in turn improves patient safety.
