Two-Year Milestones
Quarter 1:
• The 7 National Lead Centers will begin staffing, training and operating their State Lead
Center and Category of Care Management responsibilities (over the course of Q1 and
Q2)
• The National Lead Center for the Virtual Center Category of Care will begin developing
the SJB Family Health, Education and Social Registry
• All 52 State Lead Centers will begin any IRB requirements
Quarter 2:
• The remaining 45 State Lead Centers will begin staffing, training and operating their
State Lead Center and Category of Care Management responsibilities (over the course of
Q2 and Q3)
• The National Lead Center for the Virtual Center Category of Care will launch the SJB
Family Health, Education and Social Registry
Quarter 3:
• The 7 National Lead Centers will begin staffing, training and operating their Level 1
Case Management Centers (over the course of Q3, Q4, Q5 and Q6)
Quarter 4:
• The remaining 45 State Lead Centers will begin staffing, training and operating their
Level 1 Case Management Centers (over the course of Q4, Q5, Q6 and Q7)
• The 7 National Lead Centers will begin staffing, training and operating their Level 2
Case Management Centers (over the course of Q4, Q5, and Q7)
Quarter 5:
• The remaining 45 State Lead Centers will begin staffing, training and operating their
Level 2 Case Management Centers (over the course of Q5, Q6, and Q7)
• The 7 National Lead Centers will begin staffing, training and operating their Level 3
Case Management Centers (over the course of Q5 and Q6)
Quarter 6:
• The remaining 45 State Lead Centers will begin staffing, training and operating their
Level 3 Case Management Centers (over the course of Q6 and Q7)
