(Transition - LPN to RN)<br />

ASSOCIATE IN SCIENCE<br />

Program Code: 1560<br />

Program requirements are changing for the Transition<br />

LPN to RN Program (1560) effective August 20<strong>12</strong><br />

(fall term). The PROGRAM new program REVISED prerequisites must be<br />

completed (See Addendum by the end Bookmark of spring - “Revised term Programs”) for summer<br />

20<strong>12</strong> applicants starting **Contact in an August advisor.** 20<strong>12</strong>. If you have<br />

Description of Program: This one-year academic program<br />

is designed for you, if you are a Licensed Practical Nurse who<br />

wishes to become an Associate Degree Nurse (ADN). The<br />

program, by building on previously acquired knowledge and<br />

skills, provides the opportunity for upward mobility in nursing<br />

and to pursue an additional advanced degree. The additional<br />

knowledge, attitude, and skills complete the ADN requirements<br />

needed to be eligible to take the NCLEX exam for licensure as<br />

a Registered Nurse (RN). The <strong>Florida</strong> Board of Nursing has<br />

the authority to deny a license to you, if you are found guilty<br />

of violating <strong>Florida</strong> Statute 456.0635.<br />

Length: One (1) year (72 credit hours - 10 credit hours<br />

articulate with current <strong>Florida</strong> Practical Nursing licensure<br />

and competency validation by the Nursing Acceleration<br />

Challenge Exam (ACE) I - PN-RN).<br />

You should consult with a counselor/advisor prior<br />

to enrolling in this program. Admission is once each<br />

year in the second summer term for the traditional<br />

full-time program. Application packets will be<br />

accepted beginning the second week in January until<br />

the second Friday in April. Admission is once every<br />

other year in the spring term for the part-time three<br />

day per week Hardee Campus program. Application<br />

packets will be accepted beginning the second week<br />

in August until the second week in November (fall <strong>2011</strong><br />

for spring 20<strong>12</strong> acceptance, fall 2013 for spring 2014<br />

acceptance). The ADN Program is a limited access<br />

program.<br />

APPLICATION REQUIREMENTS<br />

The following materials and activities must be completed and<br />

submitted to the Allied Health advisor by the second Friday in<br />

April in order for you to be considered for admission:<br />

1. Completed SFCC Application for Admission.<br />

2. Completed Supplemental Nursing Application. (If you<br />

wish to apply to more than one nursing program, you<br />

must submit a separate application for each program.)<br />

Application materials are available in Counseling/<br />

Advising and online under Allied Health at www.<br />

southflorida.edu.<br />

3. Placement test scores from any one of the following tests:<br />

ACT, SAT I , or FCELPT (college placement test). You<br />

may schedule these tests through the college’s Testing<br />

Center.<br />

4. Official high school transcript, GED, or equivalent verifying<br />

proof of high school graduation.<br />

5. Official practical nursing school transcript.<br />

6. Official college transcript(s) verifying the completion of the<br />

following courses and all other college work attempted:<br />

The above prerequisites must be completed by the end of<br />

spring term for the traditional program. The acceptability<br />

of equivalents for any of the courses will be determined<br />

by SFCC’s dean of Applied Sciences and/or director of<br />

Nursing Education.<br />

7. Current <strong>Florida</strong> LPN license or proof of application for<br />

<strong>Florida</strong> license, if holding an out-of-state LPN license.<br />

8. Current CPR (Basic Life Care Support for Healthcare<br />

Providers) certifi cate that will not expire until completion<br />

of program, (to be photocopied by the school).<br />

9. Current IV Therapy certificate (to be photocopied by the<br />

school).<br />

10. Completed Nursing Student Checklist and submission<br />

of all related documents.<br />

11. References from the following sources may be mailed<br />

or delivered by hand if in a sealed envelope:<br />

• Current or most recent employer or instructor.<br />

• Person of your choice other than a relative.<br />

<strong>12</strong>. Completed registration form and fee for NLN Acceleration<br />

Challenge Exam (ACEI) PN to RN. All items must be<br />

forwarded to the Allied Health advisor at SFCC, 600 West<br />

<strong>College</strong> Drive, Avon Park, <strong>Florida</strong> 33825.<br />

13. Human Anatomy and Physiology I and II and Microbiology<br />

must have been taken within seven years of admission<br />

to the program. If course was completed more than<br />

<strong>12</strong>9
