Completion of the jobWhen the job has been completed all <strong>HSE</strong>-related documents must be returned to HPE.3.1.4.5. Monthly <strong>HSE</strong> reportThe HPE <strong>HSE</strong> manager must compile a monthly <strong>HSE</strong> report by the 3rd working day of the followingmonth (Appendix 11). This report will contain the following components at least: Reports on accidents and their evaluation and measures introduced to avoid them in future. LTIF: i.e. the number of accidents with more than one day's downtime per 1,000,000 workinghours. LTIR: i.e. the number of accidents with more than three days' downtime per 1,000,000 workinghours. Medical treatment and first aid incidents per 1,000,000 working hours Reports on near miss incidents and their evaluation and measures introduced to avoid them infuture. Number of safety meetings held on the site and a short summary of the topics discussed.Up-to-date document on determining and assessing risks.Number of safety inspections carried out by his own employees on site and their results andlogs. Summary of <strong>HSE</strong> corrective measures in connection with <strong>HSE</strong> checks,audits, inspections etc. including the current status. Environment-related events, such as ground and waterway contamination and measures toeliminate it etc.Amount of waste disposed of per month and cumulatively, divided into waste code numbersWaste disposal certificates for the disposal of hazardous wasteAnnual waste balance sheet bearing in mind the Waste Sorting Closed Substance Cycle WasteManagement Act.An evaluation and trend analysis using accident and near accident statistics will be performed byHPE HS.The monthly report is part of the final <strong>HSE</strong> documentation and must be given to the client when thework has been completed.3.1.5. OrganisationThe duties and responsibilities relating to <strong>HSE</strong> requirements are explained using an organisationalchart (cf. Appendix 8).HPE will compile and maintain a list of subcontractors, helped by the relevant subcontractors, inaccordance with Appendix 4 .List of all subcontractor's employees with the corresponding information on qualifications will besummarised in a qualification matrix (see Appendix 7 Qualification matrix).3.1.5.1. OBLThe OBL, who represents HPE for the assembly phase, is responsible for all <strong>HSE</strong> matters and isauthorised to issue instructions to the <strong>HSE</strong> manager.
