Monthly HSE reportThe HPE HSE manager must compile a monthly HSE report by the 3rd working day of the followingmonth (Appendix 11). This report will contain the following components at least: Reports on accidents and their evaluation and measures introduced to avoid them in future. LTIF: i.e. the number of accidents with more than one day's downtime per 1,000,000 workinghours. LTIR: i.e. the number of accidents with more than three days' downtime per 1,000,000 workinghours. Medical treatment and first aid incidents per 1,000,000 working hours Reports on near miss incidents and their evaluation and measures introduced to avoid them infuture. Number of safety meetings held on the site and a short summary of the topics discussed.Up-to-date document on determining and assessing risks.Number of safety inspections carried out by his own employees on site and their results andlogs. Summary of HSE corrective measures in connection with HSE checks,audits, inspections etc. including the current status. Environment-related events, such as ground and waterway contamination and measures toeliminate it etc.Amount of waste disposed of per month and cumulatively, divided into waste code numbersWaste disposal certificates for the disposal of hazardous wasteAnnual waste balance sheet bearing in mind the Waste Sorting Closed Substance Cycle WasteManagement Act.An evaluation and trend analysis using accident and near accident statistics will be performed byHPE HS.The monthly report is part of the final HSE documentation and must be given to the client when thework has been completed.3.1.5. OrganisationThe duties and responsibilities relating to HSE requirements are explained using an organisationalchart (cf. Appendix 8).HPE will compile and maintain a list of subcontractors, helped by the relevant subcontractors, inaccordance with Appendix 4 .List of all subcontractor's employees with the corresponding information on qualifications will besummarised in a qualification matrix (see Appendix 7 Qualification matrix).3.1.5.1. OBLThe OBL, who represents HPE for the assembly phase, is responsible for all HSE matters and isauthorised to issue instructions to the HSE manager. An essentialcomponent is safety-conscious work.3.1.5.3. IBS managerThe IBS manager represents HPE for the commissioning phase and is responsible for all HSE matters.The HSE manager supports the IBS manager.3.1.5.4. IBS FBLFBL’s are authorised to give instructions to subcontractors to carry out their duties. An essentialcomponent is safety-conscious work.3.1.5.5. HSEare differentiated (see also Appendix 8, HSE organisation organisational chart):HSE manager Organisationally the HSE manager reports to the OBL and the HSE (HS) manager. The HSE manager co-ordinates and organises the implementation of the HSE plan with regardto the planned safety and co-ordination meetings, the risk analyses, the work approval procedureand HSE documentation. Details are governed in the HPE job description (see Appendix1.). He may stop the work if there is imminent danger. Qualification in accordance with/ based on:Germany: Degree and "Specialist in Health and Safety at Work" In disciplinary matters the HSE manager is subordinate to the HSE (HS) manager and reportsto him and the OBL.HSE officer Organisationally the HSE officer is subordinate to the HSE manager. The HSE officer supervises the work particularly from an HSE point of view directly on site. Helooks at the work as a whole. Details are governed in the HPE job description (see Appendix2.). The relevant subcontractor's HSE officer inspects their own work particularly. He may stop thework if there is imminent danger. Qualification in accordance with/ based on:Germany: "Specialist in Health and Safety at Work"3.1.5.6. Co-operation between the site construction manager and HSE staffClose co-operation between the site management and the HSE staff is a basic pre-requisite for agood HSE standard on the site.HPE's or the relevant subcontractors' site management is responsible for safety, health and environmentalprotection on the site.
