3-7 Repeat screening in 6 to 112 months if no nutritional risk factors are identified.<br />

Refer <strong>for</strong> nutrition assessment if any of the following exist:<br />

Anthropometric<br />

Measure and weigh using standardized techniques and appropriate<br />

equipment*<br />

• Height or length <strong>for</strong> age less than 10th percentile<br />

• Weight <strong>for</strong> age less than 10th percentile<br />

• Weight <strong>for</strong> length (or height) less than 10th percentile<br />

• Weight <strong>for</strong> length (or height) greater than 90th percentile<br />

• BMI less than 10th or greater than 90th percentile<br />

• Change in weight or length of 2 or more percentile channels<br />

• Inadequate growth or weight gain <strong>for</strong> more than one month (under age<br />

two)<br />

Plot on standard growth charts†<br />

• Height or length <strong>for</strong> age<br />

• Weight <strong>for</strong> age<br />

• Weight <strong>for</strong> height (or length)<br />

• Head circumference (under age 3 years)<br />

• Body Mass Index (BMI) (over age 2 years)<br />

Compare current measurements to reference data and to previous<br />

measurements available.<br />

If signs of neglect or physical abuse are noted, contact Child Protective<br />

Services (CPS).<br />

When doing anthropometrics, observe <strong>for</strong> signs of neglect or<br />

physical abuse.<br />

Refer <strong>for</strong> nutrition assessment if abnormal lab values of nutritional significance.<br />

Biochemical Laboratory Data<br />

Obtain lab data from medical record, WIC program, or primary<br />

care provider:<br />

• Hematocrit (Hct) or hemoglobin (Hgb)<br />

• Other pertinent lab data: serum albumin, serum prealbumin, if<br />

available<br />

Clinical/Medical History<br />

Review past medical history and current health status and<br />

diagnosis<br />

Refer <strong>for</strong> nutrition assessment if any of the following:<br />

• Anemia<br />

• Anorexia and/or bulimia nervosa<br />

• Autism or Pervasive Developmental Disorder<br />

• Cardiac, pulmonary, or renal disease (See Chapters 16, 18,19)<br />

• Chronic constipation or diarrhea (See Chapters 6 and 7)<br />

• Chronic diseases such as diabetes, cancer, HIV/AIDS<br />

• Cystic fibrosis (See Chapter 17)<br />

• Feeding problems, poor appetite or refusal to eat (See Chapters 8 and 9)<br />

• Fetal alcohol syndrome or fetal alcohol effects<br />

• Food allergies or intolerances<br />

• Frequent or recurring infections
