GYDANT ENDOGENINES DEPRESIJAS<br />
THE EFFECT OF PROPRANOLOL ON CIRCADIAN RHYTHMS OF THE AUTONOMOUS<br />
ACTIVITY PARAMETERS DURING THE TREATMENT OF ENDOGENOUS DEPRESSIONS<br />

SANTRAUKA<br />
REIKŠMINIAI ŽODŽIAI: endogeninė depresija, propranololis,<br />
c<strong>ir</strong>kadianinė ritmika, autonominė nervų sistema,<br />
š<strong>ir</strong>dies ritmo variabilumo spektrinė analizė.<br />
DARBO TIKSLAS – išt<strong>ir</strong>ti propranololio chronobiolo<br />
ginį poveikį š<strong>ir</strong>dies ritmo variabilumo parametrų<br />
c<strong>ir</strong>kadianinei ritmikai gydant endogenines depresijas<br />
(derinant jį su antidepresantais).<br />
METODAI. Išt<strong>ir</strong>ti 56 pacientai. Vidutinis jų amžius –<br />
46,9+1,4 metų. Visiems asmenims, remiantis TLK-10<br />
kriterijais, buvo diagnozuota depresija (F 31.3-31.4, F<br />
32.0-32.2, F 33.0-33.2). Ligoniai buvo gydomi arba<br />
citalopramu (20-60 mg/parą), arba m<strong>ir</strong>tazapinu (30-<br />
45 mg/parą). P<strong>ir</strong>mą grupę sudarė 19 pacientų, kuriems<br />
antidepresantas buvo derinamas su β-adrenoblokatoriumi<br />
propranololiu (nuo 40 iki 80 mg/parą). Antros<br />
grupės 37 pacientams propranololis nebuvo sk<strong>ir</strong>iamas.<br />
Vertinant š<strong>ir</strong>dies ritmo variabilumą, taikyta spektrinė<br />
analizė. Pacientai buvo t<strong>ir</strong>iami 1, 7, 13, 19 valandomis.<br />
Kontrolinę grupę sudarė 15 psichiškai sveikų žmonių.<br />
REZULTATAI. P<strong>ir</strong>mos gydymo savaitės pabaigoje<br />
grupėje, kurioje pacientai vartojo propranololį, visos<br />
paros metu buvo konstatuojamas desinchronizacijos<br />
sustiprėjimas. Grupėje, kurioje pacientai nevartojo<br />
propranololio, naktį <strong>ir</strong> dieną jau ats<strong>ir</strong>ado resinchronizacijos<br />
požymių. Išrašymo iš stacionaro momentu<br />
didesnis neigiamas fazių poslinkis konstatuojamas<br />
grupėje, kurioje buvo vartojamas propranololis.<br />
IŠVADOS. β-adrenoblokatorius propranololis gali<br />
stabdyti resinchronizacijos procesą <strong>ir</strong> skatinti<br />
endogeninės depresijos užsitęsimą.<br />

OBJECTIVE: to evaluate the effects of propranolol<br />
on c<strong>ir</strong>cadian rhythms of the indicators of the autonomous<br />
activity during the treatment of depression<br />
applying heart rate variability spectral analysis.<br />
METHODS. 56 patients have been studied. According<br />
to ICD-10, all of them were diagnosed to have<br />
depression (F 31.3 - 31.4, F 32.0- 32.2, F 33.0 - 33.2,<br />
F 34). The patients were treated with citalopram<br />
(20-60 mg/day), m<strong>ir</strong>tazapin (30-45 mg/day). The<br />
f<strong>ir</strong>st group consisted of 19 patients treated with antidepressants<br />
combined with β-adrenoblocker propranolol<br />
(40-80 mg/day); 37 patients treated with<br />
antidepressants alone comprised the second group.<br />
Assessing the autonomous regulation of the cardiovascular<br />
system, the spectral analysis of the heart<br />
rate variability was applied. The patients were examined<br />
at 1 am, 7 am, 1 pm, 7 pm The control group<br />
consisted of 15 mentally healthy subjects.<br />
RESULTS. Following the f<strong>ir</strong>st week of treatment<br />
the strengthening of rhythms’ desynchronization<br />
in the f<strong>ir</strong>st group was observed. The second group<br />
showed signs of resynchronization in the day and<br />
night hours. During the treatment, resynchronization<br />
in the second group was more rapid and by the<br />
discharge second group showed more prominent resynchronization.<br />
CONCLUSIONS. β-adrenoblocker propranolol can<br />
slow down resynchronization of c<strong>ir</strong>cadian rhythms<br />
of parameters that reflec the autonomous activity and<br />
the sleep-wake cycle during depression treatment.<br />
