2.2.I.G Residency:<br />

The completed form is valid only for the school the student is currently enrolled, and is not<br />

transferable. This form must be completed each school year.<br />

Affidavit of Educational Guardianship<br />

An Affidavit of Educational Guardianship is designed to assist parents who, for whatever<br />

reason, have another adult see to the full time care and maintenance of their children. The<br />

Affidavit of Educational Guardianship can be used to enroll a student in the <strong>Fort</strong> <strong>Worth</strong><br />

School District if ALL of the following apply:<br />

1. The student does not reside with a parent<br />

2. The student's parent DOES NOT reside in the <strong>Fort</strong> <strong>Worth</strong> <strong>ISD</strong>. A parent/legal guardian<br />

residing in a different <strong>Fort</strong> <strong>Worth</strong> school district attendance zone than their child should<br />

request a transfer through the principal at the school in the parent/legal guardian’s<br />

attendance zone.<br />

3. The parent agrees to comply with UIL policy prohibiting a student not residing with a<br />

parent from participating in varsity activities during the current school year.<br />

This guardianship is not a legal guardianship, and may not be used as such. It is intended<br />

solely to confirm educational responsibility for a student enrolling in <strong>Fort</strong> <strong>Worth</strong> <strong>ISD</strong> schools<br />

to a <strong>Fort</strong> <strong>Worth</strong> <strong>ISD</strong> resident, other than the parent, with whom the student lives. ―Parent‖<br />

includes a guardian, conservator, or other person having lawful control of a student.<br />

The following steps must be followed to obtain an educational guardianship:<br />

Educational Guardian and Parent MUST be present to obtain the Educational Guardianship<br />

Referral Form from the attending school’s Principal<br />

1. Educational Guardian and Parent MUST provide residency verification using a utility bill<br />

(not exceeding 60 days from current date), a current lease agreement, a deed, tax record, or<br />

a notarized affidavit of residency to the Principal or Principal’s designee.<br />

2. SR01 Referral Form MUST be signed by the Principal or Principal’s designee before<br />

appearing in the Student Records Office.<br />

The board of trustees’ has the authority to provide guidelines that will enable a student to<br />

substantiate his or her residency and enable the board to determine if the student is a resident<br />

of the district. Residency is not defined by an address on a driver’s license, a signature on a<br />

lease, or the address on a utility bill. These are indicators that may expedite verifying<br />

residency, but the absence of such indicators is not conclusive that the student is not a resident.<br />

Furthermore, the fact that a student is living in a household that is leased or owned by<br />

someone outside the student’s immediate family may be an indicator that the student is<br />

homeless and entitled to admission.<br />
