II Secondary Athletic Area Rentals<br />
The following instructions are to be used to request the use of a FW<strong>ISD</strong> High School or Middle School<br />
(Secondary Campus) athletic field or gymnasium:<br />
Instructions<br />
a) The requesting organization MUST be one of the types of organizations listed at GKD (LOCAL)<br />
and provide supporting documentation.<br />
b) A Certificate of Liability Insurance must be provided with the request form. The policy must have<br />
a minimum coverage of $100,000 per occurrence and $300,000 per event and list FW<strong>ISD</strong> as an<br />
Additional Insured.<br />
c) The requesting organization must complete the Request for Use of Facilities Form – Campus<br />
Athletic in detail and send/take the form directly to the requested school.<br />
d) A Rental Fee may apply. Each campus must have a Rental Fee Schedule published on its campus<br />
webpage.<br />
e) The Request for Use of Facilities Form, Certificate of Liability Insurance, and supporting<br />
documentation acknowledging the organization is one of the types allowed in policy must be<br />
submitted to the requested school not less than 30 days prior to the date requested.<br />
f) If approved, a Rental Contract will be prepared by the school.<br />
g) If a fee is charged, payment must be received in full before the event takes place. The fee is to be<br />
paid directly to the requested school.<br />
h) Cancellation fees may apply.<br />
