Section 5: Instructional<br />

For information on special education program procedures refer to the FW<strong>ISD</strong> Special Education<br />

Operating Guidelines.<br />

5.1.I Regulations<br />

Section 504 of the Rehabilitation Act of 1973<br />

Section 504 prohibits discrimination against individuals with disabilities by school districts receiving<br />

federal financial assistance. The regulations have five (5) basic requirements:<br />

Identification and location of all eligible unserved learners<br />

Evaluation<br />

Procedural safeguards<br />

Free appropriate public education<br />

Least restrictive environment<br />

5.1.II Identification and Referral of 504 Students<br />

Principals and counselors should follow these steps for identification and referral of students:<br />

A. Establish a 504 Committee (3-5 members) composed of the Campus 504 coordinator,<br />

the principal or designee, and teacher(s) as needed. Parents should be invited to attend<br />

as a member of the committee.<br />

B. Refer all students who are believed to be in need of services under Section 504 to the<br />

committee.<br />

C. Ensure that the committee notifies the parent/guardian in writing of the school’s<br />

intent to formally review the child’s educational program, the reason for<br />

review/referral, and procedural safeguards. Obtain parent permission for an<br />

evaluation of the student.<br />

D. Complete the referral/identification process within thirty (30) school days.<br />

E. Be sure that the Campus 504 coordinator attends annual training sessions. Contact the<br />

504 Director, June Davis, at 817-814-2875, if you have questions.<br />

5.1.III Notice of Section 504<br />

Each school’s student handbook should contain the following statement:<br />

It is the policy of the <strong>Fort</strong> <strong>Worth</strong> Independent School District to provide a free appropriate<br />

public education to all students who reside in the District, without regard to handicapping<br />

condition. Inquiries concerning application of this policy may be referred to the school<br />

counselor, and/or principal or you may contact June Davis, Director, Student Support<br />

Services the administrator for Section 504, at 100 N. University Drive or call (817) 814-<br />

2878.<br />
