II. The nurse or designee (secretary, clerk, assistant principal, etc.) will:<br />

A. Send the required amount of medication for the field trip in the original container and/or any<br />

necessary equipment and supplies, a copy of the completed Medication/Specialized Health<br />

Care Permit, and a copy of the Medication/Specialized Health Care Log with the person<br />

designated by the principal to administer the medication/specialized health care procedure.<br />

B. Place any remaining medication in a medication envelope noting the student’s name, name of<br />

medication, and the number of pills on the envelope. The medication envelope will then be<br />

returned to the locked medication cabinet.<br />

C. Document on original Medication/Specialized Health Care Procedure Log the staff member<br />

that was given the medication/supplies, the time given, and the quantity of medication given.<br />

(Ex: Mrs. Jones/9:15/fieldtrip/2tabs)<br />

D. Once the medication containers/supplies are returned to the nurse/office staff, the nurse or<br />

designee will transfer the pills previously placed in the medication envelope to the medication<br />

container and return the container and other supplies to the locked medication cabinet.<br />

III. The staff member designated to administer medication on the field trip will:<br />

A. Administer the medication/specialized health care procedure at the appropriate time as<br />

indicated on the Medication/Specialized Health Care Procedure Permit.<br />

B. Document the administration of the medication/specialized health care procedure by recording<br />

the time and signing/initialing the copy of the Medication/Specialized Health Care Procedure<br />

Log or by recording the information on the original log in the nurse’s office immediately upon<br />

returning to the school.<br />

C. Upon returning to the school, the designee must immediately return the medication container,<br />

remaining medications, supplies, logs and documentation to the nurse or office staff.<br />

3.13 Procedures for Medication Errors<br />

I. A Medication Incident Report must be completed by the school nurse whenever a medication error<br />

occurs in the FW<strong>ISD</strong> and forwarded to the Director of Health Services for review.<br />

II. All medication errors committed by any FW<strong>ISD</strong> employee that result in a student being seen by a<br />

physician or in an emergency room will be referred to the Human Capital Management<br />

Department after being reviewed by the Director of Health Services. Nurses, when deemed<br />

necessary, will be referred to the Peer Review Committee according to the gravity of the error.<br />

III. Other medication errors involving nurses and health assistants that do not involve students going<br />

to a physician or an emergency room, will be processed by the Director of Health Services as<br />

appropriate for the medication error.<br />

IV. Nurses committing three (3) medication errors within a one-year period will automatically be<br />

referred to the FW<strong>ISD</strong> Peer Review Committee for nurses and to the Human Capital Management<br />

Department. The committee must review the incident and make a determination as to whether a<br />

report to the Texas Board of Nursing is warranted.<br />
