2. Transportation Incident
The name and mailing address (including state and zip code) of the Carrier Company and
of the driver.
With the exception of actual fire incidents, incident reports shall be completed immediately upon returning
to the Station. All incident reports must be completed before going off-duty. The Fire Department
Administrative Assistant shall forward all reports to the State at the appropriate time.
